Community based health center offering adult and pediatric primary care. Provides a medical home for Chestnut clients, especially those with co-morbid mental illness and/or substance use disorders. Services include:

- Basic primary care, preventive care, and prescription services.

- Screenings (cancer, communicable diseases, cholesterol, blood lead, pediatric vision, hearing, and dental).

- Annual wellness exams for all ages.

- Well child exams and immunizations.

- Lab collections.

- Obstetrical and gynecological care.

- Physicals (school, sports, employment).

Tai Chi and Bingo-cize fall prevention classes held in communities to help decrease falls in the community.

School nursing.

Substance abuse prevention through the Healthy Communities Initiative. Coalition works together to provide education and activities to reduce drug and alcohol use among youth.

Dental screenings and education offered to preschool and grade school children. Sealants and fluoride treatments also available.

Childhood and adult vaccines for all people are provided at each of Elkhorn Logan Valley Public Health Department's three locations in Norfolk, Tekamah and Wisner. Includes vaccines for those who are privately insured, Medicare, Medicaid and those who are uninsured.

Provides immunizations, dental screenings, first five care coordination services for children from birth to age 18 who meet low-income guidelines.

Additionally, early intervention services are provided to identify and address developmental delays or disabilities in infants, toddlers, and preschoolers, offering coordinated support such as developmental evaluations, individualized family service plans, and therapies including physical, occupational, speech, and psychological services. Programs also focus on early identification and treatment of emerging mental, emotional, or social challenges in children to prevent further dysfunction and promote optimal development.

Lead screening to children aged 12 months or 24 months who have not had a screening by their medical provider.

Care coordination to connect children to medical and dental homes that serve their particular needs (Care for Kids).

Participants will receive their results immediately after completing the screening. No additional information is required for the screenings. If a health problem is found, the client is referred to the nearest available health clinic.

Connects children and pregnant women living in Iowa with dental, medical, and community resources for a lifetime of health and wellness.

Also provides preventive dental care to elementary students in Iowa schools. Offering dental screenings, dental sealants, fluoride varnish, and oral health education for elementary school students.

Offers preventive and restorative dental services for eligible children. Children must be accompanied by a parent, legal guardian or designated proxy.
