Senior Care Options program available to older Nebraskans who are considering admission to a nursing home and may be eligible for Medicaid assistance. Clients are screened according to a standardized assessment process developed by the Department of Health and Human Services.

Care Managers arrange for services, which allow older adults to live as independently as possible, for as long as possible.

Housekeeping and supportive services to include grocery shopping, cleaning, laundry, transportation and personal care.

Family Caregiver Support for those caring for an aging adult or grandparents caring for a minor child. Provides information, assistance, support group, respite care, and supplemental services.

Home delivered meals are provided to homebound by reason of illness, disability or isolation. The meals meet 1/3 of the daily dietary recommended needs of adults 60 years and over and the requirements of the Older Americans Act and state and local laws.

Public benefit services, legal assistance, education, Medicare filing, and volunteer training through SHIIP.

Provides a home away from home for patients and their families when they are most in need of end-of-life care and comfort.
Offers a variety of private duty nursing services at home, provided by Registered Nurses. Services include skilled care evaluations, pre- and post-operative nursing care at home, catheter care/change/continence management, diabetic home care, patient and caregiver education, home fall prevention/safety training, pediatric home care, lab draws at home, wound care/dressing changes at home, tube feeding, G-tube care and teaching, pain management, cardio/pulmonary care, medication management and administration/injections, IV therapy management and patient instruction (central line and infusion port). Other home services as needed.

Provides long-term care to help veterans and their families when dealing with a disability, chronic (long-lasting) illness, or serious health issues related to aging. Long-term health care services include 24-hour nursing and medical care, physical therapy, and help with daily tasks like bathing, dressing, making meals, and taking medicine.

Provides staff who will help provide information, training and/or assistance to youth with significant disabilities, post high school, and individuals with disabilities living in nursing facilities with attention to deflection of people with significant disabilities at risk of entering institutions, so they may remain living in the community.

Provides hospice care. Care team includes physicians, a nurse practitioner, RNs, nursing assistants/CNAs, social workers, spiritual counselors, and trained volunteers.

Bereavement support is provided for thirteen months to loved ones of individuals who utilize Northern Illinois Hospice's services. Pet Peace of Mind Program is also available to those unable to care for their pets.

Offers medical services to the residents of Dallas County and surrounding areas. The hospital offers an Acute care and Swing bed program, Hospice Acute and Hospice Respite program, Ambulatory services, Physical, Occupational, and Speech Therapy services, Dietician Education, Diabetes Support Group, Pulmonary Rehab, Cardiac Rehab, Sleep studies, Financial Assistance, and Numerous Specialty Clinics. Also offers community education including ACLS, BLS, Spanish CPR, Walk with Ease, Better Choices Better Health, and Safe Sitter.

Offers home health aide services. Allowing clients to continue living at home, including assisted living facilities while receiving medical care. Services include personal care such as showering and bathing, nail care, shaving, oral and denture care, toileting, catheter, and ostomy assistance, exercise "buddy" for range of motion, endurance and wellness, encouraging mobility, including walking with an assistive device, nutrition assistance, including meal preparation and feeding, medication reminders and light housekeeping and chores.
