Assists seniors with Medicare enrollments and Part D changes (SHIP), Medicaid enrollment and recertification, SNAP applications, LIHEAP applications, and referrals to community programs that would be useful to seniors, including food assistance, mental health needs/counseling, transitioning to senior housing, assisted living, or skilled nursing facilities.

Peer Advocates are volunteers over the age of 60 who are available to help other people, age 60 or older, with a variety of situations, concerns, problems, or questions. Services include:

HELP with questions or just someone to talk to.

Rent Reimbursement - The Rent Reimbursement program is through DHHS. PEER assists people with filling out Online or Paper form applications. This is a one time, per year program, not a monthly or emergency rental assistance program, for those who meet eligibility requirements.

SHIIP - Can help do drug plan, MA or supplemental comparisons. As well as educate New to Medicare enrollees with process and how to get enrolled in plans.

Offers a "one-stop shop" for resources that patients and individuals in the community seeking to become Bluestem Health patients can go to apply for services without the need to travel to multiple places. Team members can assist with applying for the Sliding Fee Discount Program, enrolling in the Health Insurance Marketplace or in Medicaid, or accessing Economic or General Assistance programs. From the beginning stages of the application process through completion, our Patient Support Services team works closely with patients to help them understand what is required from each program.
