Certified nurse practitioners provide a wide variety of health care services to diagnose and treat men, women and adolescents, including: adult physical exams, behavioral health consultations, pregnancy testing and family planning, well woman exams, STD testing, HIV testing, TB screening and treatment and smoking cessation. The team also provides treatment for chronic health conditions like diabetes, cardiovascular disease and asthma.

Offers women's health services to the public. Services will include birth control options, family planning education, routine health tests (pas tests and pelvic exams), colposcopy tests to detect cervical cancer, pregnancy tests and testing for STD's.

Health care for all ages, including obstetrical, pediatrics, geriatrics, general medical care, women's health, reproductive health, prenatal, obstetrics and gynecology, and radiology/ultrasound.

Blood testing for lead.

HIV testing and counseling.

Tuberculosis prevention and case management.

Immunizations for children and adults, including flu vaccines. COVID vaccines available in office or at home.

Screening and treatment for cardiovascular diseases, diabetes, cholesterol, and depression.

Routine services such as pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns, and other on-going health problems are also routine activities at the clinic.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Home Health Care offers a skilled nursing care and home health care service. Services include: personal care; essential shopping; housekeeping; meal preparation; respite care; companionship and emotional support; personal care; transportation; medication administration and teaching; Intravenous and TPN therapy; wound care; enteral feeding; prenatal and postpartum care; phototherapy; and Telemonitoring of vital signs, weight and blood sugar readings; physical, occupational, speech, and respiratory therapies; lab testing; registered dietitian; and pharmacy consultations. Social workers available to provide assistance in identifying community resources and case management.

Counseling and support for those considering their options for an unplanned pregnancy.

Skilled nursing, including wound care, dressing changes, post-surgery follow-up, IV's, diabetic education, patient teaching, and medication review/assist with administration.

Physical, occupational, and speech therapy services.

Home care aides are certified nursing assistants providing personal care services.

Homemaker services such as light housekeeping, laundry, and meal preparation.

Public health nursing offers immunization clinics, childhood dental screenings, childhood fluoride varnishes, maternal health education and services, health promotion visits (in-home safety evaluation, well-baby checks, breastfeeding assistance), emergency preparedness, community coalition representation, referral source for community based services, disease prevention activities, and community health education projects.

Provides friendship and emotional support for anyone in a crisis pregnancy. Provides free pregnancy testing, and maternity and baby clothes. Information and referrals are given to help clients meet legal, medical, financial, housing needs, and adoption information.

Dental Clinic offers preventative dental services including: X-rays, exams, cleanings, fillings and extractions.

Pharmacy services offered to patients. Those eligible for the sliding fee program receive their medications at 30 to 50% below the retail market. Kohll's, Walgreens, Genoa, and Walmart fill all medications.

Mobile Medical Clinic services, in which a van visits different locations to see patients.

HIV/STD testing provides confidential HIV/AIDS testing and counseling.

Transportation program provides a limited number of cab vouchers.

Mental health services are provided in-house through a partnership with Lutheran Family Services of Nebraska, as well as being referred out to collaborating behavioral health agencies.

Medication assistance programs help uninsured and underinsured patients apply to various pharmaceutical companies. Staff complete applications for patients to obtain their medication at an affordable cost.

WhatsOK is a website and helpline that offers free, confidential support and resources to youth and young adults between the ages of 14 and 21 with concerns about their own or a friend's sexual thoughts, feelings, and behaviors.

Provides care coordination to families by informing them of available resources regarding their health care needs such as doctor and dental possibilities. Health program include: child and adolescent health programs, adolescent pregnancy prevention program, family planning for women and men, sexually transmitted disease information and prevention (screening) services, Hawk-I, I-Smile, lead poisoning prevention, maternal health program, WIC program, and the1st Five program.

Home Health Aides offer personal care, bathing, and light housekeeping to those who are elderly, have disabilities, or have chronic health conditions. Home health aide services help individuals maintain their safety and independence.

Parenting Support - Nurses, parent coaches/family support workers, social workers, and lactation consultants provide physician-directed home visits, prenatal and post-partum support, and health education services to vulnerable families living in poverty at risk of poor health, developmental, and educational outcomes.

Shelter Nursing - Serves as the primary public health provider in all Omaha and Council Bluffs homeless shelters, domestic violence shelters, group maternity homes, and youth living on the street. Nursing services include health assessment, education, medication compliance, and management of chronic health conditions and mental and behavioral health. Nurses work with families to address their unique needs by coordinating community resources and accessing medical services to assist with basic healthcare needs, mental health, child safety, and substance and domestic abuse.

Provides flu and immunizations (including COVID) at community and workplace clinics.

School Health Programs - Provides essential preventative health and wellness screening, as well as physical and mental health services to help ensure the overall well-being of students and staff in Omaha area schools. Nurses often provide staff training and work with parents to personalize specific care for students with special needs.

Facilitates access to home health care and hospice services to community members who are uninsured or lack resources.

A family practice clinic with physicians specializing in care for individuals and families. Services include: birth control counseling, childhood immunizations, general medical care, general physical examinations, gynecology services, confidential HIV testing, pregnancy testing, seasonal flu vaccinations, sexually transmitted disease screening, and Well Baby Care.

Family centered residential care setting provides a place of hope and empowerment for woman, pregnant and in crisis, to choose life for themselves and their unborn children. Guided by the Catholic faith, and in conjunction with multiple community partners, the program provides woman, regardless of their faith preference, with residential and aftercare pathways for attaining quality pre and post-natal care, essential parenting skills and economic security through education and employment. Housing for pregnant woman usually between 6 and 12 months. Aftercare services provided forever.

Services to runaway, homeless, near-homeless and street-dependent youth. Array of services includes shelter, street outreach, transitional living, and maternity group home.

24-hour crisis line for youth needing emergency assistance.

Emergency services include shelter program (short-term), Transitional Living Program (TLP), and Maternity Group Home (MGH) for up to 18 months. A variety of support services are provided for youth while in residence.

Require authorization of parent or guardian within 72 hours of placement for shelter placements and housing services.
