BBO-C is a statewide program (outside the City of Chicago) that provides pregnant and parenting clients with direct, comprehensive, nursing assessments and assistance to connect and engage with desired services and understand the importance of a medical home.

Nurse Navigator's provide nursing assessments and any immediate nursing interventions necessary to dyads (pregnant and postpartum individuals and their infants), helping to connect families to the needed supports available in their community during in-person visits. Nurses are required to conduct the initial pregnancy visit and initial postpartum visit for this program and are expected to follow up as indicated using clinical discretion. Non-nurse navigators may provide follow-up at the discretion of the nurse for collaborating with each family to identify and mitigate barriers to accessing desired supports and services.

Health care for all ages, including obstetrical, pediatrics, geriatrics, general medical care, women's health, reproductive health, prenatal, obstetrics and gynecology.

Blood testing for lead.

Immunizations for children and adults, including flu vaccines.

Screening and treatment for cardiovascular diseases, diabetes, cholesterol, and depression.

Routine services such as pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns and other on-going health problems are also routine activities at the clinic.

Health education integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Developmentally appropriate care and family support to prenatal mothers and low-income families with children, birth to 3 years old. Information and referral to community resources as well as health, nutritional, and social activities. Socializations are held twice a month.

Provides primary medical, dental, and behavioral healthcare to anyone who visits the clinic, without regard to insurance status or ability to pay. Services include but are not limited to well-child visits, school and sports physicals, prenatal care, chronic disease management, counseling, medication assistance recovery treatment, on-site lab, and on-site pharmacy.

Additional support includes homemaker assistance for families experiencing disruptions due to illness, disability, or other challenges, and personal care services to assist with hygiene, mobility, household tasks, and nutritional needs for individuals such as elderly people, people with disabilities, or recently discharged hospital patients.

Infrastructure building activities include partnerships with tobacco control community, connections with Quitline, health equity, distribute information on IA maternal mortality, referrals for prenatal care (medical/dental), partnering with breastfeeding community, linking persons to breastfeeding resources, provide information to persons interested in breastfeeding, work with community of importance of safe sleep, connect families with resources for cribs, education on importance of oral health during pregnancy and working with dental/medical provides.

Provides well-baby care services, including health screenings and immunizations, to support the physical well-being and healthy development of infants and toddlers, with some programs extending to preschool-aged children.

Healthy Pregnancy Program services may be offered to clients during WIC appointments, in their home, or at a local public health agency. Clients can receive Healthy Pregnancy Program services in addition to other programs, such as local home visiting programs. Healthy Pregnancy Program home visits are done by a nurse who can do a physical assessment and check on the baby.

The Promise Community Health Midwifery Team offers care that is tailored to the needs of a patient's growing family and is designed to support them through the changes experienced throughout life.

Midwives are highly-trained healthcare professionals who provide many healthcare options for women. Certified Nurse Midwives offer services including fertility counseling, birth control counseling, gynecologic exams, breast exams, prenatal care, home-birth delivery, postpartum care, and more.

Provides comprehensive care and treatment for women of all ages, including high-risk obstetrical care, reproductive medicine/infertility, women’s cancers, prenatal appointments, genetic counseling, routine and annual gynecological care, and comprehensive diagnosis and treatment of gynecologic disorders.
