PACE (Program of All-inclusive Care for the Elderly) organization offering a complete personal health plan for eligible individuals. Supports living in the client's own home and community for as long as possible.

Participants receive all the care and services covered by Medicare and Medicaid, as authorized by the Interdisciplinary Team, as well as additional medically necessary care and services not covered by Medicare and Medicaid.

Provides the following services: home health care (skilled nursing, home care aids, homemakers), immunizations, dental services, lead poisoning prevention, lead testing, physical therapy, speech therapy, and occupational therapy. Additionally, the program provides communicable disease information and control.

Provides an alternative for patients who need skilled care, but do not require 24-hour supervision. Services are provided to patients under the direction of the patient’s doctor.

Services Include:

- Skilled Nursing Care Provided by a Registered Nurse and Licensed Practical Nurse.

- Pediatric Care.

- Home Health Aide Care.

- Medical Social Worker.

- Occupational Therapy.

- Speech Therapy.

- Physical Therapy.

Home health care services including skilled nursing, rehabilitative therapies, home care aides, speech therapy, dietitian consult, certified wound ostomy, continence nurse, medical alert systems and homemaker assistance.

Provides services to individuals who have conditions such as congestive heart disease, kidney disease, diabetes, muscle-nerve problems or respiratory diseases and are able to be discharged from a hospital or nursing home but need additional care at home. Require short term assistance at home because of same day or outpatient surgery or maternity-related incapacity.

Offers nurses, home care aides for personal cares and homemakers for homemaking needs for those who need assistance related to a medical diagnosis which prevents them from being able to manage these functions independently. Health Services no longer provides acute home care services funded by insurances but will provide all similar services as needed to safely assist persons to maintain independence and thus prevent unnecessary institutionalism. Offers home care, public health, immunizations, mental health administration, early childhood development, and environmental health services. Provides information on communicable diseases, routine general physical examinations to assess overall health and detect abnormalities, and birth control services to help individuals prevent conception

Provides prenatal care to ensure the health and well-being of pregnant individuals and their developing children through risk assessment, routine checkups, prenatal testing, and guidance on pregnancy-related concerns.

Provides well-baby care services, including health screenings and immunizations, to support the physical well-being and healthy development of infants and toddlers, with some programs extending to preschool-aged children.

Also offers comprehensive preventive, diagnostic, and treatment services for sexually transmitted infections, including contact tracing to help prevent the spread of infections.
