PACE (Program of All-inclusive Care for the Elderly) organization offering a complete personal health plan for eligible individuals. Supports living in the client's own home and community for as long as possible.

Participants receive all the care and services covered by Medicare and Medicaid, as authorized by the Interdisciplinary Team, as well as additional medically necessary care and services not covered by Medicare and Medicaid.

Services include but are not limited to:

Nursing Home Care

Provides home health care services which are provided by registered nurses and home health aides. Services include catheter care, IV treatment and oxygen/respiratory therapy; and in-home therapy including physical therapy, occupational therapy, and speech therapy.

Provides health-related services, such as administering oral medications, help with simple prescribed exercises, keeping patient’s rooms neat, and help patients to move from bed, bathe, dress and groom

Immunization clinic.

Health screenings.

In-home skilled nursing.

Home health aides.

Flu clinics in fall and winter months.

Foot care clinics monthly, or by appointment.

Offers public health programs and services, including bioterrorism and county disaster preparedness. Provides mammograms and Pap tests for early cancer detection, lead poisoning screenings for young children, and home health aide services to support recently discharged hospital patients, elderly individuals, and people with disabilities. Works closely with the county Board of Health and serves as a funding source for community needs assessments. Also provides information about communicable diseases.

Provides 24-hour home health services for individuals in need. Services include on-call nurse support, arrangement of equipment for use in the home, assessment of medical conditions. blood draws, communication with healthcare providers, disease management and education, evaluations, management of medical supplies, medication management, ostomy care and teaching, review of insurance benefits and filing of claims, safety home evaluation, speech therapy, occupational therapy, and physical therapy. 

Provides CNA's and or Home Health Aids in multiple environments to assist with bathing, dressing, errands, companion care, and much more; LPN's assist with med set-up/supervision, dressing changes, and other services; technology is offered in the form of medication dispensers and various types of sensors.

Provides in home medical services. Services include but not limited to skilled nursing, medication management, therapy (OT, PT, ST), and medical social worker services.

Home health care and hospice services.

Provides continuing care services at home that focus on wellness, independence and asset protection. We’re a non-profit organization helping older adults thrive. This is a membership program with monthly fees. Monthly membership fees guarantee at home care services for life. Works to keep the client in the home as long as possible.

In-home skilled nursing, case management, and therapies (physical, occupational and speech) with 24/7 on call availability.
