Provides home care services for individuals transitioning home from the hospital or rehabilitating after an injury or surgery. Services include skilled nursing care, infusion and IV therapy, physical therapy, speech therapy, occupational therapy, home medical equipment, wound management, and pediatric care. 

Group homes of four to eight individuals with 24/7 staff support. Dietician-directed meal program tailored to individual needs. Also offers assistance for individuals who live in their own home or apartment but also need help every now and then.

Offers to assist with bathing, personal care, exercise, shopping for food, clothing, meal planning, preparation, house cleaning, laundry assistance, and money management.

Provides services to individuals who have conditions such as congestive heart disease, kidney disease, diabetes, muscle-nerve problems or respiratory diseases and are able to be discharged from a hospital or nursing home but need additional care at home. Require short term assistance at home because of same day or outpatient surgery or maternity-related incapacity.

Provides an alternative for patients who need skilled care, but do not require 24-hour supervision. Services are provided to patients under the direction of the patient’s doctor.

PACE (Program of All-inclusive Care for the Elderly) organization offering a complete personal health plan for eligible individuals. Supports living in the client's own home and community for as long as possible.

Participants receive all the care and services covered by Medicare and Medicaid, as authorized by the Interdisciplinary Team, as well as additional medically necessary care and services not covered by Medicare and Medicaid.

Nursing Home Care

Provides health education to the public and to schools. Helps with planning and preparedness for infectious diseases or other public health incidents for the Decatur County area. Offers immunizations to children, adolescents and adults, including the flu and COVID vaccines. Home health care services and home nursing is provided to those in need. Can provide personal care assistance for bathing, hair care, housekeeping and laundry, meal planning and preparation, errands, and waiver services.

Home health care services including skilled nursing, rehabilitative therapies, home care aides, speech therapy, dietitian consult, certified wound ostomy, continence nurse, medical alert systems and homemaker assistance.

Registered nurses, aides, physical, occupational, and speech therapists, and social workers. Medication management, disease management, wound care, patient and family education related to current conditions and chronic illnesses, assistance with personal care, and more. Hearts at Home (cardiac care, including congestive heart failure), diabetes management, infusion nursing, and fall prevention. In-home telemonitoring is available to monitor patient's blood pressure, pulse, and weight and oxygen, as ordered by the physician.

Offers a registered nurse who provides the following in the person's home; health education, medication management, health assessment, and maternal or child health care. Calhoun County Public Health is not a certified home health provider and does not provide Medicare or Medicaid eligible services.
