Group homes of four to eight individuals with 24/7 staff support. Dietician-directed meal program tailored to individual needs. Also offers assistance for individuals who live in their own home or apartment but also need help every now and then.

Offers home health aide services. Allowing clients to continue living at home, including assisted living facilities while receiving medical care. Services include personal care such as showering and bathing, nail care, shaving, oral and denture care, toileting, catheter, and ostomy assistance, exercise "buddy" for range of motion, endurance and wellness, encouraging mobility, including walking with an assistive device, nutrition assistance, including meal preparation and feeding, medication reminders and light housekeeping and chores.

Offers skilled nursing, physical therapy, occupational therapy, speech therapy, home health aids, homemaker assistance services, and companions available to assist persons to remain safely in their homes.

In home health care services, including skilled nursing, physical, occupational, and speech therapy, and home health aides.

Specialized nursing services include IV infusion therapy, wound care, tube feedings, men, orthopedic nursing, and care for people with chronic diseases, such as heart failure, diabetes, or COPD.

Rehabilitation services.

For Veterans who need assistance with activities of daily living (such as dressing, grooming, bathing, eating, toileting, mobility, etc.) or caregivers experiencing burden. Veterans can decide what mix of services will best meet their needs and hire their own personal care aides.

Provides an alternative for patients who need skilled care, but do not require 24-hour supervision. Services are provided to patients under the direction of the patient's doctor.

Offers a skilled nursing care and home health care service. Services include: personal care; essential shopping; housekeeping; meal preparation; respite care; companionship and emotional support; personal care; transportation; medication administration and teaching; Intravenous and TPN therapy; wound care; enteral feeding; prenatal and postpartum care; phototherapy; and Telemonitoring of vital signs, weight and blood sugar readings; physical, occupational, speech, and respiratory therapies; lab testing; registered dietitian; and pharmacy consultations. Social workers available to provide assistance in identifying community resources and case management.

Provides in home medical services. Services include but not limited to skilled nursing, medication management, therapy (OT, PT, ST), and medical social worker services.

Hospital, acute care, emergency care, swing bed, skilled, and intermediate level care hospital (17 beds).

Extended care facility which provides long term care.

Home health services include nursing, nurse aids, physical therapists, speech therapists, and occupational therapists.

Nephrology and diabetes consultation via telehealth.

Nutrition counseling services.

Physical, vascular, pulmonary, and cardiac rehabilitation.

CPR classes.

Support groups related to Alzheimer's caregivers and Parkinson's Disease.

Provides education to those who have been diagnosed with diabetes.

Provides home health care services for individuals needing assistance with recovery, injury, and allowing to teach patients and their families about their conditions and how to manage at home. Services include registered nurses, home care aides, medical social workers, physical therapists, occupational therapists, speech therapists, wound, ostomy, and continence nurses.

Offers public health programs and services, including bioterrorism and county disaster preparedness. Provides mammograms and Pap tests for early cancer detection, lead poisoning screenings for young children, and home health aide services to support recently discharged hospital patients, elderly individuals, and people with disabilities. Works closely with the county Board of Health and serves as a funding source for community needs assessments. Also provides information about communicable diseases.

Provides health education to the public and to schools. Helps with planning and preparedness for infectious diseases or other public health incidents for the Decatur County area. Offers immunizations to children, adolescents and adults, including the flu and COVID vaccines.

Home health care services and home nursing is provided to those in need. Can provide personal care assistance for bathing, hair care, housekeeping and laundry, meal planning and preparation, errands, and waiver services.

Provides options to help seniors stay in their homes as long as possible. Meets with families to do in-home assessment to determine what services may be needed. Services may include help with meals, home delivered meals, handyman services, personal care, transportation or medical or legal issues.
