Home Health Care and Hospice services available through community partners.

Companion care services include social interaction, medication reminders, meal preparation, shopping and errands, light housekeeping, pet care, laundry, incidental transportation, etc.

Personal care includes ambulation assistance, bathing and showering, continence care and toileting, skin and hair care, dressing, feeding, etc.

Specialty care/Skilled nursing care includes wound care, medication administration, catheter care, etc.

Provides home care services to individuals with a serious illness, regardless of age or stage of disease. Supportive care is provided in-home by a nurse, who works with other healthcare providers. Services can be provided along with healing treatments including nursing, physical therapy, occupational therapy, speech therapy, social services, and home health aides.

Offers home health care services. Will make home visits. Immunizations for communicable diseases for all age persons, health promotion and education. Also home care nursing, personal home care aide with limited homemaking services.

Offers home health aide services. Allowing clients to continue living at home, including assisted living facilities while receiving medical care. Services include personal care such as showering and bathing, nail care, shaving, oral and denture care, toileting, catheter, and ostomy assistance, exercise “buddy” for range of motion, endurance and wellness, encouraging mobility, including walking with an assistive device, nutrition assistance, including meal preparation and feeding, medication reminders and light housekeeping and chores.

Provides in home support services for individuals in need. Services include personal care, light housekeeping, meal preparation, exercises and a respite care program.

Provides continuing care services at home that focus on wellness, independence and asset protection. We’re a non-profit organization helping older adults thrive. This is a membership program with monthly fees. Monthly membership fees guarantee at home care services for life. Works to keep the client in the home as long as possible.

For Veterans who need assistance with activities of daily living (such as dressing, grooming, bathing, eating, toileting, mobility, etc.) or caregivers experiencing burden. Veterans can decide what mix of services will best meet their needs and hire their own personal care aides.
