Provides health care services to those experiencing acute or chronic pain and/or illness, or those seeking medical services that are not due to an emergency. Services include annual screenings or check ups, chronic disease management, immunizations, pregnancy tests, pre and postnatal care, school and sports physicals, sexually transmitted infection tests, and well-child exams.

Specialist referrals for services not available at the clinic are made for all patients.  Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

- Home Health Aide: Provides individuals an opportunity to maintain safety and independence in their home environments, delaying or preventing the need for transitions into a higher level of care such as a skilled nursing facility.

- Parenting Support: Offers home visits, educational opportunities, and community referrals so parents are empowered to maintain self-sufficient, healthy, and happy families.

- Shelter Nursing: Nurses visit homeless clients at shelters, crisis centers, and through outreach on the streets. Provides health screening, assessment, and referrals with the intent to ensure individuals and families residing in shelters have access to comprehensive health, community, educational and social services.

- Flu and Immunization Program: Provides immunizations clinics in the community, corporate sites, senior living centers, and drive through in Omaha, Lincoln, Council Bluffs, and surrounding communities.

- School Health: Provides health screening and assessment, referral, and follow-up; immunization compliance; physical and vision exam compliance (kindergarten, seventh grade, and new students); development of student health and emergency plans; communicable disease control; handling of sudden illness and injury; training of first responder teams and those giving medications; and health policy development.

Home Health Care | York General Health Care Services

Private duty home care services include skilled nursing, bath aide, venipunctures, foot care, and adult immunizations.

Emergency Response Service provides a necklace or wristband for assistance at the push of a button; responders and/or emergency personnel available 24 hours every day. $38 one-time installation fee, $30 per month fee.  Fee paid if person qualifies for a Medicaid waiver.

Immunization clinic held the 2nd Monday of every month at West View Medical Building, 2319 N. Lincoln Avenue.  A $15 donation is requested for children vaccinations 0-18 years.  Call for current fee for adult vaccinations.  Adult vaccinations include Hepatitis A, Hepatitis B, Tetanus, Influenza, meningococcal, and pneumonia.

Provides health care services in the home by registered nurses. Services include infusion therapy, central venous catheter care and injections, wound and catheter care, gait training, and health education. Also offers physical, occupational, and speech therapists, medical social workers, certified nursing assistants, and telehealth nursing.

Pediatric Home Nursing: Provides care according to the patients’ needs and the doctor’s orders, including both extended stay and skilled visits. Provides in-home care for complex pediatric clients such as those using ventilators, IV and TPN needs, trachs, g-tubes, central lines, respiratory medication, and monitoring equipment.

Consultative services are provided with care centered on the needs of the patient and family, providing counseling about the disease and assistance in identifying the patient's personal goals for care.
