IHH brings together mental, physical and social health services into one integrated program. Each family is connected to a three-person team that includes a nurse, a care coordinator and a family peer support specialist who provide flexible support focused on individual and family strengths, needs and comfort level.

Provides the County Rural Offices of Social Services, which works to improve health, hope, and successful outcomes for the adults in the region who have a chronic mental illness, intellectual disability or developmental disability. Support services are offered for those who are disabled. The County Rural Offices of Social Services is the only place to complete an enrollment packet for the disability program. A service coordinator will schedule an intake interview within 10 days of receiving contact for assistance to assess service and support needs.

The IHH team also works with healthcare providers to coordinate care, achieve health goals, and improve wellness.

Works with other providers to ensure services are well matched to help meet the child's emotional, mental, and physical health goals.

Integrated Health Home services are those services provided to adults with serious mental illness (SMI), and Medicaid coverage.


Services are classified as whole person care, for physical health, mental health, and lifestyle choices that impact overall wellness. Services are provided by an Integrated Health Home Team which includes a nurse care manager, a care coordinator, and peer support specialists.


Integrated Health Home providers help with the following:

-- Manage physical and behavioral health services;

-- Create a whole health plan for medical care and wellness goals;

-- Focus on choices, strengths, good health, and total wellness;

-- Help with transportation to healthcare appointments if needed;

-- Use technology to link all your services together; and

-- Provide peer support services to encourage recovery and resiliency.

Offers the PACT (Program for Assertive Community Treatment), which provides comprehensive, community-based psychiatric treatment, medication management, rehabilitation and support to individuals possessing a serious and persistent mental illness. Services are provided 24 hours a day, 7 days a week. The PACT team seeks to address all aspects of an individual's functioning and to help nurture life goals and integration into the community.

The mental health team works with patients to assess current coping skills, address emotional needs, and work through lifelong patterns. A board-certified psychiatric mental health nurse practitioner provides specialty care in the areas of psychiatric consultation, evaluation, and psychiatric medication management. These services are provided on-site by the mental health team.
