Provides assistance to adults with serious mental health conditions who are Medicaid beneficiaries in coordinating and managing their health and mental health care services. The program provides support and assistance for clients in identifying goals and working with the healthcare team to ensure that the needs of the client are met. Services include coordinating the client's primary and specialty care providers, coordinating the care provided in the hospital, clinic, and the community, and access to a Peer Support Specialist who have experienced living with mental illness.

IHH brings together mental, physical and social health services into one integrated program. Each family is connected to a three-person team that includes a nurse, a care coordinator and a family peer support specialist who provide flexible support focused on individual and family strengths, needs and comfort level.

Offers the PACT (Program for Assertive Community Treatment), which provides comprehensive, community-based psychiatric treatment, medication management, rehabilitation and support to individuals possessing a serious and persistent mental illness. Services are provided 24 hours a day, 7 days a week. The PACT team seeks to address all aspects of an individual's functioning and to help nurture life goals and integration into the community.

Offers the FACT (Forensic Assertive Community Treatment) program, which provides services to individuals in the criminal justice system with a serious mental illness. FACT works to help people live successfully in the community while avoiding a return to jail, prison, or unnecessary psychiatric hospitalizations. Services are provided 24 hours a day, 7 days a week. The program focuses on person-centered treatment planning and operates as a mobile outreach team - meeting clients in the home or community to provide services.

Integrated Health Home services are those services provided to adults with serious mental illness (SMI), and Medicaid coverage.

Services are classified as whole person care, for physical health, mental health, and lifestyle choices that impact overall wellness. Services are provided by an Integrated Health Home Team which includes a nurse care manager, a care coordinator, and peer support specialists.

Integrated Health Home providers help with the following:

- Manage physical and behavioral health services;

- Create a whole health plan for medical care and wellness goals;

- Focus on choices, strengths, good health, and total wellness;

- Help with transportation to healthcare appointments if needed;

- Use technology to link all your services together; and

- Provide peer support services to encourage recovery and resiliency.

Provides care coordination, advocacy and support for individuals and families. Team helps to manage whole health including physical, behavioral, and social health services.

Provides outpatient mental health therapy, medication management, and psychiatric assessments. Services are offered to adults with disabilities, including individuals with Mental Illness, Intellectual Disability, or other Developmental Disabilities, who require support in multiple areas.

The program integrates Medicaid funding and county regional funding to assist individuals in transitioning back into the community after hospitalization for acute or chronic mental or emotional disturbances.

Eligibility requires qualification for Polk County Regional funding; individuals may or may not be eligible for Medicaid.

Transitional Mental Health Services

Provides a program to take care of individual's bodies and minds by making sure doctors work together. A team of regular doctors and mental health providers helps each individual get the care needed. The goal is to make it easier for individuals to get help to stay healthy, feel better, and recover. It also makes sure individual's with mental health or substance use problems can get the same kind of help as those with other health problems. All doctors and providers work as a teak so care is organized.

Provides a comprehensive and integrated range of mental health, substance use, and primary health services to any individual, regardless of funding. Integrates mental health and substance use care with physical health care, utilizing a wide variety of evidence-based practices. Responsible for directly providing (or contracting with partner organizations to provide) nine required types of services: crisis mental health services; screening, assessment and diagnosis, including risk assessment; patient-centered treatment planning; outpatient mental health and substance use services; primary care screening and monitoring of key health indicators/health risk; targeted case management; psychiatric rehabilitation services; peer support and family supports; and intensive, community-based mental health care for members of the armed forces and veterans.

Offers a holistic approach that ensures participants receive comprehensive and integrated care including the mind-body connection, while addressing their support needs.

Each participant is assigned a team of professionals including a Registered Nurse, Care Coordinator, and Peer Support (adult) and Family Peer Support Specialists (children) who actively review the member's mental and medical condition and create an individualized care plan. Provides assistance in navigating the healthcare system and accessing resources, personalized supports, and a major focus on wellness, prevention activities, education, and healthy lifestyle choices.

Offers a comprehensive mental health center that provides outpatient individual, family, and group psychotherapy. Services include mental health evaluations to screen, diagnose, and plan treatment for individuals experiencing acute or chronic psychiatric issues. Psychiatric case management is available to assess needs, coordinate services, and monitor progress to ensure effective care.

Integrated Health and Community Outreach Programs are offered, including Jail Diversion programs, Integrated Health programs, and Support Community Living programs. Life skills education is provided to help individuals develop the knowledge and skills necessary for independent living, addressing areas such as job search, money management, nutrition, home management, and health care.
