Wraparound is a philosophy of care with defined planning process used to build constructive relationships and support networks among students and youth with emotional or behavioral disabilities (EBD) and their families. It is community based, culturally relevant, individualized, strength based, and family centered.

Serves boys and girls ages birth to 17 who may be at risk of out-of-home placement due to disruptive behavior in school, at home, or in their community. Caseworkers connect families with community resources by researching the best community options to meet their individual needs.

Community Circle of Care (CCC) is a regional System of Care program providing care coordination for community-based services and supports to non-Medicaid children and youth with serious behavioral or mental health needs in northeast Iowa. CCC serves children and youth up to 21 years old. The CCC program provides wraparound services to ensure that youth and families are connected to: assessment and diagnosis, care coordination, and social supports.

The Pediatric Integrated Health program (PIH) provides care coordination and family support services to Iowa children and youth who are eligible for Medicaid and have mental or behavioral health diagnosis. The PIH care coordination team helps children and families get the care they need by working together with the child's doctor, school, therapists, and other service providers. PIH Family Navigators have firsthand experience as the parent or primary caregiver of a child with a mental or behavioral health challenge. PIH Social Workers work with families to identify goals, develop care plans, and connect them to community-based services and resources. PIH Nurses help families manage their child's medical needs.
