CSA's Integrated Health Program provides whole health coordinated care for youth and adult, age 14 and older, with a serious mental illness or serious emotional disturbance who are Medicaid eligible. This program is designed to address the unique mental health, physical, and social needs of young people and their families as they transition into adulthood.

Provides remedial services that offer in-home skill development services to children who are eligible for Title 19 and have a mental health diagnosis to enhance functioning. Services provided to children and families include; conflict resolution, problem solving, anger management, coping, social interaction, self control, crisis intervention, and communication skills.

This program includes the following services: counsel individual on relationship building, problem solving, better communication and coping skills to reduce conflict and other emotional issues while strengthening confidence.

Non-medical care includes companionship, light housekeeping, organizing, medication reminders, laundry, meal preparation, transfer assistance including bathing and dressing. Also provides pediatric care, veteran services, medical social worker, behavioral health.

One hour minimum per visit, 24/7, or as little as once per week. Services may be provided in the home or in a facility.

Individual and Family sessions focus on positive communication, problem solving, and skill development. The staff of Sky Ranch will meet with the client weekly in the comfort of their home or in the Sky Ranch office. The primary focus will be on individual support and guidance in the areas of behavior, interpersonal relationship skills, internal control development, andfamily relationship skills, etc.

Offers the following Community Services:

Home and Community-Based Services (HCBS) provides services for adults with intellectual or mental disabilities, a qualifying brain injury or handicapping condition. Individualized support is provided in the client's home or other community-based settings and is available 24 hours.

Home-Based Habilitation Services (HBHS) are designed to assist individuals with serious and persistent mental illness with the acquisition, retention or improvement in skills related to living in the community. Services assist in the daily living needs of the client and can be provided 24 hours a day.

Community Support Services (CSS) provide integrated services and supports to persons with a serious mental illness. Interdisciplinary teams provide assessments and service planning, monitoring of mental health symptoms, medication management, problem-solving assistance, coordination of appointments and transportation and support.

The program blends Medicaid funding and county regional funding. Persons in this program may or may not be eligible for Medicaid. Persons in this program need to be eligible for Polk County Regional funding.

Emergency protective custody for those age 19 and over, involuntary. Evaluation and treatment of acute symptoms, initiated by law enforcement personnel in Region 3.

Day psychiatric program, adult partial hospitalization.

Psychiatric home health provides in-home nursing assessment, occupational therapy to assess functional skills, psychology to assist with behavior plans for caregivers, and social work to provide in-home psychotherapy.

Offers senior's an In-home health/mental health assessment, in-home mental health treatment, case management, and referrals to appropriate community resources. The Outreach Team consists of clinical social workers and a consulting psychiatrist to provide appropriate support services.

In-home skill-based services for children.
Offers in-home counseling and skill development to families with children ages 6-17 who reside in the Quad-County Area in southeast Iowa. The child can have no involvement with DHS or JCS. Services last up to six months and include a family team meeting at which time the family plan is developed.
