Senior Care Options program available to older Nebraskans who are considering admission to a nursing home and may be eligible for Medicaid assistance. Clients are screened according to a standardized assessment process developed by the Department of Health and Human Services.

Services may include offering information and guidance about Medicaid eligibility, application processes, managed care options, covered benefits (including long-term care and home and community-based services), co-payment procedures, Medicaid spend-down requirements, and the connections between Medicaid and Medicare. Assistance may also include addressing questions about Medicaid services for individuals with disabilities, enrollment support, and referrals to Medicaid-approved providers.

Assists Nebraskans who are age 60+ or have a disability (and/or their representatives) to allow the person to live as independently as possible in the setting of their choice.

- Assists in accessing resources based on the person’s needs and preferences, including information and referrals for services such as housing, personal care, transportation, food and nutrition, public benefits, therapies, employment, etc.

Provides assistance with filing of Benefits Access Application, Medicare Part D, and general information and referral to other services for seniors and individuals with disabilities.

Helps seniors, individuals living with a disability, and their families by providing referrals to organizations and agencies for needed services and programs.

Offers 24-hour intakes by phone at the local number and the state hotline. A caseworker will make a home visit to investigate any allegations of abuse and assist the victim. Interventions will be offered to the victim and referrals made to the appropriate organizations. Interventions can include but are not limited to in-home services, legal, medical, relocation, and counseling.

Investigates allegations of abuse, neglect, and exploitation of older adults age 60 and older and persons with disabilities (ages 18-59) who live in domestic (non-institutional) setting. Appointed by the Illinois Department on Aging to investigate allegations of physical and emotional abuse, financial exploitation, confinement/neglect, deprivation of food or medication, sexual abuse, and self-neglect

Assists seniors with Medicare enrollments and Part D changes (SHIP), Medicaid enrollment and recertification, SNAP applications, LIHEAP applications, and referrals to community programs that would be useful to seniors, including food assistance, mental health needs/counseling, transitioning to senior housing, assisted living, or skilled nursing facilities.

Helps applying for benefits, information on community resources, and assistance to caregivers and grandparents raising grandchildren.

Answers questions regarding government benefits and community resources such as: Social Security, Medicare/Medicaid, Benefits Access Application (driver's license discount, seniors and persons with disabilities ride free), energy assistance (LIHEAP), intervention and advocacy on a client's behalf, assessment of need and referral for services, and assistance in the completion of forms and applications for benefits.

Provides Care coordinators trained by the Illinois Department on Aging that can meet with individuals and their families in the home. This is a free consultation to provide counseling and education about available senior services and then arrange and link the older adult to programs that are needed. 

Provides public health services to the residents of Boone County. Services include blood pressure screening, case management for the Frail Elderly, communicable disease control, care for yourself program (cancer screening program), emergency preparedness planning, immunization clinics, communicable disease control links, and Iowa Department of Public Health Centers for Disease Control and Prevention.
