Free, voluntary program where coaches work with families for 3-6 months to help navigate and access resources in the community, increase families' protective factors, and problem-solve to address challenges a family may be facing. A coach is assigned to meet with the family and connect them to needed support.

​The program offers case management support, mental health services for parents/guardians and children, education support, and some supports to meet basic needs and ensure safety.

​Other support includes assistance finding safe, stable housing, connections to local food resources, help applying for benefits and tracking application status, budgeting and employment assistance, and education supports for parents/guardians and children.

Provides financial assistance for basic needs (food, gas, rent and utilities), transportation assistance (car repairs, gas and paying for public transportation) offering coaching and family goal-setting support, and facilitating access to early childhood resources. Referrals for mental health and counseling services.

Provides one-on-one support throughout pregnancy.

Offers personalized support for a healthy pregnancy, full term delivery, and postpartum care through the baby’s first 6 months.

Navigators (who are nurses and social workers) can also connect clients with community resources and help find medical care.

Those who qualify for the program may receive:

- Personalized prenatal and postpartum education.

- Community referrals and links to services.

- Services provided free of charge.

Coordinates support for individuals and families whose needs cannot be fully addressed through existing community resources.

- Does not provide direct emergency assistance. Instead, navigators work alongside referring agencies to identify available community resources, coordinate partner support, explore flexible funding when appropriate, and connect individuals with longer-term coaching or services when needed.

Works with pregnant woman, or families with a high risk infant, or a foster child under 6, to obtain health care services or other necessary services needed to have a healthy pregnancy and to promote the infant or child’s healthy development. Services include health counseling, evaluation of medical and social needs, referrals to needed services or supports, high risk infant follow-up for those in need and DCFS medical case management.

Emergency shelter for survivors of domestic/sexual violence & homelessness with their minor children needing a safe place to stay. There is a 40-bed shelter for victims and their children. A case manager will work with clients while they are at the shelter to make a service plan for them and their children. Residents are provided room and board with meals, 24-hour case management, counseling, and material assistance. This location is one of the coordinated entry points for anyone who is homeless in the area. 

Provides advocacy, education, support, information and referral, and coaching services for families and children. Helps children during family separation by supporting placement in a safe, stable environment and encouraging a smooth return home when possible. Teaches families about laws, court processes, and case resolution. Offers one-on-one coaching to explain the rights, roles, and responsibilities of parents. Equips parents with age-appropriate skills to create safe, loving, and nurturing homes. Provides education, resources, and support to help prevent domestic violence, child abuse, and neglect. Offers domestic violence awareness education to help the community understand the effects of domestic violence on children, families, and victims.

Offers case management services for families with children under the age of 17 to assist in overcoming challenges in their lives. Case worker's help assess the needs of the family, resources are shared and create an action plan to help the family map out goals and the ways to achieve them, and works with the family to find the resources they need to be successful.
