Offers to connect pregnant women and infants to health and wellness services to reduce infant mortality and decrease morbidity, prematurity and low birth weights.

Women assessed to be at high risk of a poor birth outcome based on a previous poor birth outcome and/or medical and social risk factors associated with premature births, low birth weight, and infant/maternal complications are offered the Better Birth Outcomes program (BBO), a more intensive case management program with monthly visits, a home visit each trimester and transportation assistance, as needed, to access prenatal and postpartum care.

BBO can assist pregnant women in applying for Medicaid and can refer to an OB/GYN for prenatal services. Case Managers for BBO programs complete referrals to needed community resources, provide advocacy and care coordination between participants and physicians and provide support to encourage healthy behaviors.

Helps support the healthy development of babies during pregnancy by identifying risks and providing a support system. The Nine2Thrive project model is a public - private partnership bridging primary care and public health services. Through screenings, prenatal healthcare providers refer pregnant individuals to Nine2Thrive for free, confidential connection to support. Individuals can refer themselves to the Nine2Thrive program, but their primary OB Physician must be in Polk County or the South-Central Iowa Medical Clinic in Corydon. Support Specialists from Nine2Thrive link pregnant individuals to community and healthcare resources. The Nine2Thrive Support Specialists follow up with the referring healthcare provider. Community supports individuals may be linked to include mental health supports, financial assistance, parenting support and education, child development information, other individualized supports to meet the family's needs.

Family Case Management assists with understanding the importance of prenatal care, finding a doctor for prenatal care, understanding proper nutrition for both mother and child, understanding the stages of child development, learning parenting skills, recognizing the signs and symptoms of preterm labor, understanding the importance of well-child visits and immunizations, and connecting to additional resources. 

Public Health Nurses and Case Managers provide services in the office or at home, offering information on prenatal needs, infant care, parenting, and household safety tips to protect infants and children. Additionally, guidance is available on child passenger safety, including selecting an appropriate car seat based on a child's height and weight, correct car seat installation, and ensuring proper child restraint in vehicles.
