Provides a wide variety of independent living services for individuals with disabilities with the objective of helping them to function more independently in family and community settings and to secure and maintain appropriate employment. - Bogard Individual Service Coordination: Ensure active treatment for seniors in nursing facilities. This service is for individuals from a closed lawsuit in 1988. No new individuals are eligible for this service.

- Case Coordination: Information and referrals; community awareness and education; enrollment of individuals on the PUNS statewide waitlist for services.

- Individual Services and Support Advocacy (ISSA): Assistance with accessing services to meet the individual's needs; act as advocates for the individual.

- PACE will readily provide sign language interpreters to access PACE events on request.

Offers service coordination for persons with developmental disabilities, including comprehensive assessments to identify developmental delays and disabilities. Provides life span individual service coordination and assists the individual and family by conducting a pre-screening to determine need, building relationships with staff, case coordination/management, finding specialized services for individuals with disabilities, and information and referral. 

Developmental assessments may involve evaluations of cognitive functioning, communication skills, independent living abilities, social and emotional development, and motor functioning, conducted by a team of professionals to recommend appropriate treatment and care.
