Works with primary care providers who identify social/emotional or developmental concerns, family stressors, or caregiver depression during well child medical exams. If a condition is identified, the family is referred to a support specialist who provides care coordination. Specialists may provide referrals for developmental screening, mental health or behavioral health services, or other needs, and will monitor the family's progress.

Participants will practice skills, play learning games, and do family projects in highly interactive sessions. Humanely and effectively reducing Des Moines’ outdoor community cat population by altering, vaccinating, ear-tipping and returning cats to the areas where they are already living and thriving. The program is free to community cat caregivers in Des Moines.

Provides psychiatric evaluation, medication assessment, medication education and training, and on on-going medication management. The psychiatric team works in collaboration with clients primary care physician, therapist, case manager, and care coordinators to ensure effective and coordinated care.

The program will assist customers needing health or dental assistance, up to $300, with vision screenings, prescription payments, prescribed lift chairs, hearing aid assistance, dental cleanings/services, etc.

Investigates allegations of abuse, neglect, and exploitation of older adults and persons with disabilities who live in domestic (non-institutional) setting.

Once a report has been filed, a caseworker will make a home visit to investigate any allegations of abuse and assist the victim. Interventions will be offered to the victim and referrals made to the appropriate organizations. Interventions can include but are not limited to in-home services, legal, medical, relocation, and counseling.
