Grants may be available for home or car repairs, replacement of clothing or food, and for the expense of temporary housing. Original receipts are required for those seeking reimbursement for actual expenses related to storm recovery.

ELIGIBILITY CRITERIA:

- Must live in Governor Disaster Declaration area.

• Must be a legal resident of the United States and at least 18 years old
• Income guidelines for families, income must be at or below 200% of federal poverty level.
• Individuals residing in the household must be able to verify occupancy.

COUNTIES ELIGIBLE FOR ASSISTANCE: The Governor has issued a disaster proclamation for the following counties in Iowa:

- Adams, Dubuque, Madison, Mills, Montgomery, Page, Pottawattamie, Ringgold, Taylor, Union, and Wapello Counties are now eligible for the Individual Disaster Assistance Grant and Disaster Case Management through the local community action agency in response to a severe weather incident (proclaimed June 12, 2026). Deadline to apply for the grant is July 27, 2026 (45 days from proclamation).

- Cass and Decatur Counties are now eligible for the Individual Disaster Assistance Grant and Disaster Case Management through the local community action agency in response to a severe weather incident (proclaimed June 16, 2026). Deadline to apply for the grant is July 31, 2026 (45 days from proclamation).

- Polk, Osceola, Jasper, and Story Counties are now eligible for the Individual Disaster Assistance Grant and Disaster Case Management through the local community action agency in response to a severe weather incident (proclaimed July 6, 2026). Deadline to apply for the grant is August 20, 2026 (45 days from proclamation).

Care coordination team works together with families to develop a plan of care that addresses a child’s medical, social, behavioral, educational and financial needs. Care coordination is free for all children who have eligible medical conditions, regardless of their family’s income level.

Care coordination can include finding specialized medical care, helping families understand their child’s diagnosis and medical treatment plan, explaining insurance benefits and attending school meetings.

Helps children who are medically fragile remain in their family home rather than a hospital or skilled nursing facility.

Assists with:

- Finding specialized medical care

- Making sure families understand their child’s diagnosis and medical treatment plan

- Helping families understand their insurance benefits and maximize their coverage

- Attending IEP /504 Plan school meeting

- Connecting families with financial support, grants and other community resources

- Preparing for the transition to adulthood

Oversees the election process, voter registration, maintains financial records for the county, processes passport applications, maintains current real estate information and assessment maps.

Offers the education and support families need so every child is equipped for kindergarten through Early Childhood Education.

Provides early childhood education services (childcare and Pre-K), with a focus on the development of emotional and social skills and school readiness. Services are available at two sites.

Care team that specializes in providing integrated primary medical and behavioral health care and social services to the LGBTQ+ community. Services provided:

- Assessments and referrals to Hormone Replacement Therapy.

- Letters for surgery.

- Counseling.

- Family Medicine.

- Case management and care coordination.

- Nutrition education.

- OB/GYN.

- Enrollment assistance (insurance/ Medicare/Medicaid).

- Referrals for other needed services.

Offers assistance for older adults, who might otherwise need nursing home care, to remain in their own homes by providing in-home and community-based services. Services include comprehensive care coordination, adult day services, in-home services (cleaning, meals, laundry, shopping/running errands, and personal hygiene), emergency home response service,  and automated medication dispenser service.

Can also provide temporary respite care services or supervision of a functionally impaired person, allowing the primary caregiver time away to complete other tasks. 

Provides programs in character and leadership development, education and career development, health and life skills, arts, sports, fitness and recreation. Offers structured programming promoting academic success, healthy lifestyles, and character development – STEM, physical fitness activities, service to others, arts and crafts, etc.

Offers a drop in center that provides a range of services to assist individuals with housing, employment, and other essential needs. Case managers provide support with housing applications, employment applications, resumes, and obtaining necessary documents such as photo IDs, social security cards, and birth certificates.

Also features community shower facilities and offers specialized services such as Veteran’s Assistance and Christ-centered counseling. Additionally, the center provides clothing assistance, including general clothing items and winter-specific clothing such as coats, gloves, hats, and scarves, depending on availability.

Offers HIV, Hepatitis C, Syphilis, Gonorrhea, and Chlamydia testing to all individuals

Offers both short and long-term experiences for youth. Short-term experiences include afterschool programs, camps, school-sponsored activities, conferences, and other events. Long-term experiences include learning communities in agriculture such as county clubs.

Helps patients understand their medications and vaccinations and be able to obtain them affordably.

Provides community housing services to individuals 18 years of age or older diagnosed with a psychiatric disorder who require a supervised or supported living environment for community residency. Program services focus on improving functioning, increasing independent living skills, and other supportive measures according to each individual’s unique needs.

The Community Housing Program consists of five residential facilities, with individual apartments, each of which may be furnished depending upon the availability of funding: Sterling Apartments, Country Lane Apartments, Harvest House, Curry Lane Apartments, and Randolph Lane Apartments.
