Provides financial and support services to Veteran families that are homeless or imminently at risk of being homeless. The goal is to stabilize the families in housing, while providing on-going support as needed to sustain independent living. Services may include rental and utility deposits and limited assistance, some moving costs and emergency supplies, case management services, transportation assistance, assistance in obtaining VA benefits and other public benefits.

By utilizing Central Navigation, the goal is to coordinate existing resources and match participants with a resource to either solve an immediate need or develop a longer-term coaching relationship.

When funding is available, may provide help with a security deposit or a first month's rent to McHenry County residents who have been unable to save for their move due to no fault of their own.

SSVF is a Veterans Affairs funded program which is delivered through Primary Health Care. This program is focused on helping homeless and unstably-housed Veterans and their families living in Warren, Polk, Dallas, Jasper, Story, and Marshall Counties. Services provided include:

CASE MANAGEMENT services that will assist participants in obtaining VA and other public benefits, including: health care referrals, daily living referrals, personal financial planning, fiduciary and payee referrals, legal services, housing counseling services, employment and training referrals.

TEMPORARY FINANCIAL ASSISTANCE (TFA) which may include rent assistance, moving expenses, security and utility deposits, transportation, and child care. All financial services based on eligibility and available funding.

Coordinated Entry is the front door to homeless services. It is the first point of contact for those who need housing assistance. It provides guidance to anyone experiencing homelessness to the correct support and housing programs most appropriate to their situation.

PROACTIVE AND SHORT-TERM PROGRAMS
STREET OUTREACH: collaborate with local partners to meet individuals and families experiencing homelessness where they are and offer assistance in camps and on the street.
LIBRARY OUTREACH: regular hours at libraries in Ames, Boone and Marshalltown where people experiencing homelessness can meet with a case worker and work on a housing plan.
EMERGENCY SHELTER: short-term emergency shelter and housing support services for individuals and families experiencing homelessness. Provides food, financial education, and health and wellbeing assistance.

LONG-TERM PROGRAMS
SUPPORTIVE HOUSING: client-focused services based on the degree of need and the severity of barriers individual/families are facing. Will help clients secure affordable housing and provide the tools needed to overcome financial barriers and maintain a permanent home. Individualized support services, including life skills counseling, employment assistance, budgeting and financial literacy, and community resources referrals are also provided.
RAPID RE-HOUSING: is a time limited program, with temporary financial housing assistance and supportive services for people experiencing homelessness leading to permanent housing and self-sufficiency. The timeframe will be from a minimum of one month to typically a maximum of six months of rental assistance.
CONNECTION TO COMMUNITY SERVICES: connects clients to various external programs and community resources to help them achieve sustainable, healthy outcomes.

Provides low-income clients with rent and security deposit assistance to help a household maintain housing. To qualify for this program the client needs to be behind on their rent and the landlord has to have served them a legal eviction notice. The client will also be required to have some type of income (employment, social security, TANF, unemployment, etc.) to qualify for assistance. Babysitting (or some other type of working) for cash will NOT be accepted as income. A client can only be assisted every two years through this program.

Transitional Housing is designed to move homeless families from streets and shelters into temporary housing and to maximize self-sufficiency, this program is for families only and they must be homeless in order to receive assistance. Homeless means living on the street, in an emergency shelter, or a place not meant for human habitation (cars, parks, sidewalks, abandoned building, etc.) Occupancy is limited.

Supportive Housing is designed to move homeless families/individuals with documented disabilities from streets and shelters into temporary housing and to maximize self-sufficiency. Enrollment is limited based on availability.

Homeless prevention, rental counseling, housing assistance and advocacy to assist persons to avoid homelessness and locate permanent housing.

ELIGIBILITY:
-- Household must have eviction notice (rent) or shut-off notice (utilities - no funds for Black Hills Energy)

-- Household must be at or below 30%-50% of area median income

-- Household must meet both of the following conditions:
No appropriate subsequent housing options have been identified
AND
The household lacks the financial resources and support networks needed to obtain immediate housing or remain in its existing housing (i.e. without this assistance, the household would become homeless)
