Medical transport includes doctor’s appointments, non-emergency hospital visits, hospital discharge, dialysis, physical therapy and stroke rehabilitation, pulmonary and cardiac rehabilitation, and dental appointments. 

Non-medical transport includes long-distance trips, airports, train or bus stations, special and family events, sports events, shopping, recreational or social events, trips to and from employment, and other similar activities. Door-to-door or curb-to-curb service is available for individuals who otherwise would have no means of transportation.

Residential services include Traditional Group Homes, Community Support Program, and Supported Community Living Services.

Traditional Group Home offers training in self-help skills, household management and leisure-time skills. These programs integrate people into the community in the least restrictive environment. All individuals are encouraged to have an outside activity. Group home sites are in Red Oak, Malvern, and Shenandoah.

Community Support Program teaches independent living skills to persons living in their own apartment or home. Individuals receive additional support services as needed such as transportation, personal shopping, meal planning, socialization, self-help skills, household management skills, and money management.

Supported Community Living Program provides up to 24 hours of support to assist individuals with their daily needs.

Respite Care Program provides short-term relief in the absence of the family or caregiver, such as a vacation or a night out.

Paratransit transportation for Siouxland area including, but not limited to, individuals with disabilities:

-- Individuals must complete an application for assistance and 21 days must be allowed to process the application.
-- Door-to-door service is available and costs $3.60 each way.
-- If the individual can get to the curb, then curb-to-curb service costs $3.60 each way. Siouxland Regional Transit System (SRTS) is a demand-responsive, ADA accessible, public transit system which provides public transportation services to the general public of Cherokee, Ida, Monona, Plymouth and Woodbury counties in Iowa and Southern Union County in South Dakota. Buses are equipped with a mobility device lift.

-- Curb-to-curb is defined as the individual waiting at the curb when the vehicle arrives and the individual is dropped off at the curb.
-- Door-to-Door service is defined as the individual waiting outside the door, and will be delivered to the door at his/her destination.
-- Each rider is allowed one (1) personal care attendant, and this person will ride free of charge. The attendant must be assisting the client or will be charged for the trip. Please schedule the additional person when making the reservation.

Determines client eligibility for the City of Lincoln's special transportation services including Senior Saver, a discount for persons, 62 years or older, on regular bus routes; Go For Less, a discount for persons with disabilities on regular bus routes; the Handi-Van, which offers door-to-door transportation for persons unable to handle a fixed-route bus due to their disability; and the Taxi Coupon Program, which offers a reduced taxi fare for persons eligible for use of the Handi-Van. In addition, mediation is provided for StarTran and their customers regarding services.

DART Paratransit's Bus Plus service provides transportation for people who have a disability that prevents them from using fixed-route bus service. All Paratransit service is door-to-door and wheelchair accessible.

Eligibility:
Bus Plus is DART’s ADA door-to-door complementary service for riders with a disability that keeps them from being able to use Fixed Route services. Riders must apply to use the service. Once approved, riders may travel within the Bus Plus zone for any reason and must pay a $3.50 fare for each ride they take.

To start the eligibility process, fill out a DART Bus Service Eligibility Form. This form is available at www.ridedart.com/services/paratransit, and must be submitted to DART's Paratransit Department, which will review the application and mail back the results within 21 days. An application can also be mailed to you by calling DART Customer Service.

Reservations:
Once approved, requests for service can be made by calling DART Paratransit at 515-283-8136 Monday through Friday between 8 am-5 pm. Reservations for Sunday and Monday travel can be made on Saturday and Sunday between 8 am-4 pm by calling (515) 283-8147 and leaving a voicemail message with the client name, contact number, and trip details. DART staff will return the passenger's call to schedule a trip. Reservations will be accepted up to seven (7) days prior to the trip date.

Polk County Service Details:
The partnership with Polk County allows low-income disabled citizens 18 and older to apply for service for trips to medical appointments, the grocery store, and a once-a-month miscellaneous trip. Income eligible dialysis patients who have no transportation may also apply. To check on eligibility and/or apply for this service, call Polk County (515) 286-3484. If approved, the customer rides DART Paratransit at no charge. The funding agency may restrict the rider to certain types of transportation, such as medical appointments, work or grocery shopping. Polk County Adult Services does not fund employment or school transportation.
