Offers lead testing to children ages 6 and younger, which helps identify children who may be at risk for lead poisoning. Also provides information on prevention, intervention, and appropriate renovation that will reduce potential lead hazards.

In-office 15-minute blood lead testing available for children ages 6 months to 5 years of age. Blood lead testing promotes early detection and referral for treatment of lead poisoning; informs parents and guardians about their children's exposure to lead; and promotes the importance of living in a lead-safe environment.

Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns, and other on-going health problems are also routine activities at the clinic.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Offers a variety of public health services, including communicable disease control, tap water testing, pool sanitation, public health facility inspection, tuberculosis screening, immunizations, and more.

Administers the WIC program. Participants in WIC receive an eWIC card for healthy foods. The foods provided by WIC provide nutrients that are essential for good health and growth. Participants also meet with nurses and dietitians to review health and diet information, discuss important nutrition information for their family, and receive referrals for other services when needed.

HAWK-I Coordination provides health care coverage for Iowa children in families with limited income.

Child care nurse consultation with child care providers to improve health and safety practices.

Lead screening to children aged 12 months or 24 months who have not had a screening by their medical provider.

Care coordination to connect children to medical and dental homes that serve their particular needs (Care for Kids).
