Co-pay relief program (CPR) currently provides direct financial support to insured patients, including Medicare Part D beneficiaries, who must financially and medically qualify to access pharmaceutical co-payment assistance. The program offers personal service to all patients through the use of call counselors, personally guiding patients through the enrollment process.

Eligibility Requirements:
  • Patient must be currently insured and have coverage for the medication(s) for which they seek financial assistance
  • Patient must have a physician confirmed diagnosis of the disease under which they are requesting financial assistance
  • Patient must reside and receive treatment in the United States
  • Patient's income must fall at or below the income guidelines of the fund under which they are requesting financial assistance. All funds have income guidelines of either 300% or 400% or less of the Federal Poverty Guideline (FPG) with consideration for the Cost of Living Index (COLI) and the number in the household
  • Patient will be informed immediately upon application if they qualify for assistance

Affordable, comprehensive primary health care especially for those with limited resources. Includes general physical exams and immunizations.

Services are provided regardless of ability to pay through a sliding fee discount program. Fees are calculated based on household size and documented annual income. All patients are eligible to apply for the sliding fee discount, even those with insurance.

Dental Clinic offers preventative dental services including: X-rays, exams, cleanings, fillings and extractions.

Pharmacy services offered to patients. Those eligible for the sliding fee program receive their medications at 30 to 50% below the retail market. Kohl's, Walgreens, Genoa, and Walmart fill all medications. Mobile Medical Clinic services, in which a van visits different locations to see patients.

HIV/STD testing provides confidential HIV/AIDS testing and counseling.

Transportation program provides a limited number of cab vouchers.

Mental health services are provided in-house through a partnership with Lutheran Family Services of Nebraska, as well as being referred out to collaborating behavioral health agencies.

Prenatal care for women.

Radiology has certified x-ray technicians to provide limited x-ray services.

Medication assistance programs help uninsured and underinsured patients apply to various pharmaceutical companies. Staff complete applications for patients to obtain their medication at an affordable cost.

Translation services offered to non-English speaking patients. Languages include Spanish, Vietnamese, Arabic, Kurdish, Burmese, Karen, and Thai.

They are able to provide assistance in connecting to needed community programs and services. In addition, base on funding and donations, the office can support a crisis fund which allows them to provide assistance with rent, medical and prescription expenses, gas money, utilities, and other basic necessities when the household is facing an emergency situation. The agency actively seeks funds for the crisis program and all funds donated to a county stay in that county and are used to support the residents of that county.

PREVENTION AND TESTING: free HIV and STD testing. Free safer-sex supplies are available as well as bleach kits for sterilizing needles. Pre-Exposure Prophylaxis/Post-exposure Prophylaxis education and access (medications that significantly reduce the risk of becoming infected when taken as directed).

MEDICAL CARE: Comprehensive primary care and HIV specialty care.

FINANCIAL ASSISTANCE offers assistance when other community resources are exhausted. People living with HIV may qualify for emergency assistance from this fund. This service is intended to be temporary financial assistance.

CASE MANAGEMENT will work with clients individually to determine their needs, may assist clients in applying for these programs: Housing Assistance, Transportation, AIDS Drug Assistance Program,Disability, Medicaid, Food Stamps,General Relief, WIC, FIP, and others. Call for details.

MENTAL HEALTH PROGRAM provides individuals living with HIV/AIDS culturally relevant psychotherapy in a non-judgmental, accepting environment by a therapist experienced in the complex issues associated with HIV/AIDS.

SUPPORT & COUNSELING GROUPS for people living with HIV, persons testing HIV positive, family and friends. Call for information on meeting times and locations.

Funds assist Tribal members in payment of health care when no other source is available, or supplements other resources when they are exhausted. Payer of last resort for up to two primary medical visits per year, specialized services, vision services, emergency services, prescriptions (in some cases), dental services, annual required physicals for youth, and immunizations. Elderly ages 65 and over are eligible for all services listed regardless of their coverage.
Limited emergency financial assistance available on a case-by-case basis for needs such as medical, prescription, rent, and utilities. Provides referrals to other agencies when unable to assist.

Community Response is designed to reduce unnecessary involvement of child welfare and juvenile justice while increasing the informal and community supports for youth and families. By utilizing Central Navigation, the goal is to coordinate existing resources and match participants with a resource to either solve an immediate need or develop a longer-term coaching relationship.

Application approval is required to receive benefits. Payments are made directly to landlords and/or service providers. No cash benefits are available. Application decisions are guaranteed within 30 days or less of applying.

Program benefits no more than one time every six months. Applicants must include household information on their application. No utility payments are authorized during the November 1 to April 1 disconnection moratorium.
