Provides home visits 1-2 times a month to do medication set-up, health assessments, blood draws for lab tests and other health interventions.

Also offers the Lifeline personal emergency response system units to individuals and families. The Medical Alert Button provides access to trained care specialists 24 hours a day, seven days a week, 365 days of the year. 

Provides access to a 24 hour helpline with care coordination specialists offering resource referrals, case management and support to Illinois Veterans, family members, or caregivers experiencing emotional challenges associated with PTSD, mild TBI, Military Sexual Trauma (MST), stress, anxiety, depression, addiction and substance abuse. 

Free programs assist adult cancer survivors and caregivers:

-- Survivorship 101: For cancer patients and their caregivers, a 9 week program is available in person or over Zoom in small group settings by professional facilitators. Provides survivorship education, emotional/mental health support and rehabilitation. Survivors connect with and provide support to each other.

--Under 45 Survivorship 101: Same program as above, but tailored especially for the under 45 age demographic.

-- Brain Fog Program: A 6 week program over Zoom by professional facilitators in small groups for survivors post-cancer treatment. Brain exercises, memory support tools, and research-proven methods for mental stability for both cancer and Covid survivors.

Provides the starting point for obtaining information and arranging services or referrals for individuals and their families ages 60 and older. Counselors connect individuals with resources in the community, providing information about a broad range of community services available, and to share state or federal program resources. Services include information on senior programs, caregiver issues, home health care services, Medicare and Medicaid, senior housing options, and transportation options.

Offers a support group for caregivers.

Provides dementia care for patients and their caregivers. 

Offers Child Care Expense Assistance through the Iowa Department of Human services is available to the children of income-eligible parents who are absent for a portion of the day due to employment or participation in academic or vocational training or PROMISE JOBS activities. Assistance may also be available for a limited period of time to the children of a parent looking for employment. APPLY TO BE A REGISTERED CHILD CARE PROVIDER. Offers services for homeless youth and their families. Helps students enroll or stay in school. Connects families to community resources for health, transportation, food, and more.

Offers support with gender marker changes; helps clients with changing of the name and sex on their legal papers to their chosen name.

Empowers low income individuals and families in need with some food items to cook a Thanksgiving meal in their own home rather than eating at the Open Door Mission on Thanksgiving Day. This also includes extra groceries to help during the Holiday season when school is out. There will be only 1 Turkey 'N Fixins program per address.

For Thanksgiving 2025:

Beginning November 1st, 2025, community members can come to the Timberlake Outreach Center or the Council Bluffs Outreach Center (on the days they are open) and receive a turkey or a ham, along with some other traditional Thanksgiving specific foods as a part of their once a month shopping time. (Only one turkey or one ham per address.)

Program runs through Nov 27th, 2025. Individuals/families can receive Turkey 'N Fixins Wednesday-Saturday at the Timberlake Outreach Center, and on Tuesdays only at the Council Bluffs Outreach Center.

Individuals/families must have a current piece of mail in order to participate in the Turkey N Fixins program.

Provides the Unemployment Compensation Insurance programs and assists with questions regarding and applications for unemployment compensation, and provide resources for those seeking employment and retraining information.

Offers a commuter bus service going from Cedar Rapids Ground Transportation Center with stops in Kirkwood Cedar Rapids Park and Ride Lot, Coralville Iowa River Landing District, University of Iowa Hospitals and Clinics, Downtown Iowa City-Court Street Transportation Center. Handicap accessible.

Provides health care services to those experiencing acute or chronic pain and/or illness, or those seeking medical services that are not due to an emergency. Services include annual screenings or check ups, chronic disease management, immunizations, pregnancy tests, pre and postnatal care, school and sports physicals, sexually transmitted infection tests, and well-child exams.
