In-home and community-based program designed to reduce youth and family risks that contribute to delinquent behavior while increasing youth and family protective factors.

- Family Consultants work with families to improve youth and family functioning in the home while preventing the need for youth to be detained or referred to an out-of-home setting.

- Each family has an individualized service plan developed that addresses emotional, behavioral, social and educational needs of at-risk youth and families.

- Services provided to the youth and family include: teaching cognitive behavioral techniques and skills that promote self-sufficiency, developing problem-solving skills, practicing new skills, and developing skills to help communication.

Offers to assist a relative caregiver who wants to obtain guardianship through probate court of a relative's child living in the home of the caregiver. This includes

- Educating and helping the caregiver negotiate the probate court system.

• Assisting caregivers to obtain required documents.
• Arranging for legal counsel in counties where legal counsel is necessary.

  EFSP caseworkers meet with the relative caregiver monthly until guardianship has been obtained or until other services have been provided. These services can include crisis intervention, applying for benefits, working with local schools for the child's enrollment, and referral to a range of needed services that the caregiver may have to help meet the basic needs of the family.

Provides therapy and counselling services to adolescents, adults, families, and seniors. If medication is indicated, psychiatric services will be recommended to ensure the individuals treatment is cohesive and optimal to their recovery.

Provides outpatient services for individuals with substance use and mental health disorders. Includes counseling services.

Provides a continuum of mental health treatment, supports and habilitation services designed to assist adults with a serious mental illness to overcome barriers and develop a happy and meaningful life. CBRS staff are committed to helping individuals who desire increased satisfaction in life areas including home, work, school and relationships. Services provided: First Episode Recovery Support Team (FERST), Program for Assertive Community Treatment (PACT), Supported Community Living (SCL), Community Support Services (CSS).

Services for individuals who need help due to a health condition, physical impairment, or are homebound for other reasons. Services include: transportation to non-emergency medical appointments, building wheelchair ramps, modified steps, steps and railings, and running errands or grocery shopping.

Provides intake and assessment for households experiencing homelessness in Champaign County, IL. Based on eligibility and priority, CIH links households experiencing homelessness with shelter, transitional, and permanent housing program options when there are program vacancies. In addition, CIH can make referrals to local community resources based on the needs of individuals served. Individuals and families can request a CIH at partner agencies.

Provided to people with brain injuries and their family members.

Food pantry offering frozen meat and bread. Other items available as donated.

Provides support, empowerment, and recovery to survivors of sexual violence (and the people that love them). Counselors assist in the process of recognizing how sexual violence has impacted the survivor's feelings, relationships, self-esteem, and ability to function while working with them to find healing.

Survivors may choose to receive individual, family, couple, and/or group counseling. In addition, support groups are offered to adult survivors of childhood sexual abuse, teens, and parents.

Provides Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) services in Black Hawk, Bremer, Buchanan, and Grundy Counties. Services include nutritious foods to supplement diets, nutrition education, breastfeeding promotion and support, and referrals to health care and other community resources.

Provides a monthly basket for food for income-eligible seniors.

Manages and maintains all trial court records; process fines, fees, and court costs owed to the state; child support checks and civil judgments owned to litigants; and maintain record of liens on property.
