Provides health care services to those experiencing acute or chronic pain and/or illness, or those seeking medical services that are not due to an emergency. Services include annual screenings or check ups, chronic disease management, immunizations, pregnancy tests, pre and postnatal care, school and sports physicals, sexually transmitted infection tests, and well-child exams. 

Foster Grandparents provide service to children with special needs in schools, Head Start or Health facilities. Volunteers who are age 55 and older and meet income guidelines agree to serve a minimum of 10 hours per week and receive a tax-free stipend of $4/hour and other benefits.

Senior Companion Program provides older adults with special needs assistance in their homes. Volunteers who are age 55 and older and meet income guidelines agree to serve a minimum of 10 hours per week and receive a tax-free stipend of $4/hour and other benefits.

Offers workshops that teach individuals and families how to get more out of money and how to save money for financial goals. This is a series of workshops that are a blend of instruction and hands on activities so each concept can be mastered. 

Provides a variety of residential programs for those who have a developmental, vocational, emotional, mental, or physical disability. Programs include the following types of residential services:

- Community-Integrated Living Arrangement (CILA): a living arrangement provided to Adult Waiver participants in a group home, family home or apartment where eight or fewer unrelated adults with developmental disabilities reside under supervision of the community developmental services agency. Residents receive a comprehensive individualized array of residential options, personal support services and supports under the direction of a community support team within the local agency. Funding comes through DHS Division of Developmental Services.

- 24 Hour CILA: these homes have staff on-site at all times Individuals are present. Staff provide both scheduled and unscheduled supports and services as needed by Individuals.

- Intermittent CILA: serves Individuals whose service plan documents that they do not require 24-hour staff presence, and documents the situations in which individuals may safely be away from staff direct supervision. Intermittent Residential services in apartments or family homes have staff available on call 24 hours per day. On-site staff are available to provide both scheduled and unscheduled supports and services as needed by the Individuals served and as specified in each Individual’s service plan.

- At Home Day Program: part of the residential program for Adult Waiver participants. At Home Day Programs are based in the individual’s CILA home and provide a structured personalized program of in-home and community activities for Individuals who are unable to participate in out-of-home day programs.

- Group Home: a living arrangement provided to participants in a group home, family home or apartment where eight or fewer unrelated adults with mental illness and /or developmental disabilities reside under supervision of the community services agency. Residents receive a comprehensive individualized array of residential habilitation, personal support services and supports under the direction of a community support team within the local agency. Funding usually comes through DHS Division of Mental Health.

- Apartments: a living arrangement where the person has their own relationship with a landlord and is basically living independently. Some supervision can be provided if the person wishes the assistance. Case management and case coordination services are always available on a as needed basis.

- Own Home: a living arrangement where the Individual owns their own home and is living independently on their own or with family. Some supervision can be provided if the person wishes the assistance. Case management and case coordination services are always available on an as-needed basis.

Public library services, including internet access.

Provides rental units for low-income individuals.

Provides financial assistance to individuals with disabilities and the agencies that serve them in Jo Daviess. The foundation requires the submission of grant applications in order to disburse funds. Immediate need funding requests may be submitted directly to the Executive Director or the Grant/Gifting Chair of the Hope Foundation of Jo Daviess County.

Requests for grants must be related, but not limited to:

- Health.

- Housing.

- Transportation.

- Education.

- Employment.

For the 2025 holidays:

Food baskets provided in December (delivered Sat, Dec 20, 2025). Call Sat, November 8, 2025, 9 am-2 pm to register. Call on this date only.

Assists individuals with mental illness, intellectual disabilities, and developmental disabilities in accessing needed services and supports. Coordinators provide information, referral, assess individuals for needs, develop a treatment plan, and coordinate funding as needed.

Provides assistance to individuals and families who are facing a crisis situation with food and/or shelter. Homeless prevention services can assist families who are facing eviction or utility disconnection in preventing their family from becoming homeless. Crisis funds may be available for one month’s rent, utilities, or groceries for a short period of time. This assistance is intended for individuals and families who need temporary, one-time assistance to resolve an emergency situation.

Provides sober living/transitional housing services for single men. Individuals are allowed to reside in the program as long as they need continued support and are in compliance with policy and procedure. Individuals must complete one of the emergency shelter programs to be admitted into the Walters Sober Living program. 

Offers a book giving program that mails free books to children from birth to age five. To help increase literacy skills, encourage school readiness, and engage children in one of the most fundamental skills necessary to succeed in life – Reading!

The books will arrive about 8-10 weeks after registration has been approved.

Helps increase access to education and employment, among other supports.

Services include seeking expungements and sealing criminal records, restoring driver’s licenses, and removing other barriers to employment, education and housing, SNAP and TANF denials, calculations, overpayments and sanctions, special education, school discipline, and school enrollment issues, Community Care Program and Home Services Program issues, and tax disputes with IRS, including innocent spouse relief, identity theft, and collections.

Intensive case management for those in need of mental health and/or substance abuse services.

Hospital providing acute care, skilled care, respite care, 24 hour emergency room, outpatient surgical procedures, on-site lab, CT scanner and other traditional radiology services, physical therapy 5 days a week, occupational therapy, speech therapy, MRI, Doppler/Echo, ultrasound; bone density testing, immunization clinic, foot clinic. Specialists include: orthopedics, dermatology, and podiatry.

Meals on Wheels.

The agency is responsible for tax collection for state and local governments, regulating the manufacture, distribution, and sale of alcoholic beverages, and overseeing local property tax assessments. It also serves as the funding agent for the Illinois Housing Development Authority. Additionally, the agency provides access to tax forms, including federal and state tax forms, property tax relief applications, rent rebate and renter's tax credit applications, property value reassessment applications, utility tax exemption applications, and other related documents, which are available in physical locations or online for download.
