Information: member inquiries, member enrollment, Iowa Care, billing issues, disease management or maternity management. *To apply for Medicaid contact local DHS office. CONSUMER CHOICE OPTION PROGRAM gives Iowans on Medicaid the freedom to choose service providers to assist them with their unique needs. To participate in the program, you must be receiving Medicaid funding through a Home and Community Based Services (HCBS) waiver and be referred by a case manager.
