Offers workshops that teach individuals and families how to get more out of money and how to save money for financial goals. This is a series of workshops that are a blend of instruction and hands on activities so each concept can be mastered. 

Offers mental health services to individuals, groups, families and couples. Services include mental health evaluations, therapy, web therapy, diabetes focused CBT therapy, anger management and Children in the Middle classes.

Offers free support and services for unplanned pregnancies including, pregnancy testing, ultrasound, STD/STI testing and treatment.

Maintains county parks and recreation areas and provides hunter safety classes, camping, fishing and boating information. Outdoor recreational opportunities are available through county parks and outdoor wildlife areas.

The Community Health Representatives provide the following services:

- Case finding/screenings.

- Case management.

- Home visitation services.

- Blood pressure and blood glucose screenings.

- Patient care services and other patient services.

- Consults (medication, medical orders, appointments).

- Environmental health.

- Homemaker services.

- Community development.

- Health education.

- Local transportation.

Offers law enforcement services to the Henry County area. Issues gun/firearm permits, runs requested background checks, and manages the county Jail.

Provides weatherization services to low income households. Helps conserve fuel and reduce energy costs by making homes and apartments more energy efficient. Also provides many health and safety upgrades.

Services vary, but may include insulation, furnace work, and air sealing.

Provides temporary homes for children of families in crisis. Services allow parents an opportunity to resolve a temporary crisis while reducing risk of abuse, neglect, and isolation for children. Connects families with a family friend, who offers a healthy relational support as the family navigates the situation. Also connects the family with a family coach, who can provide linkage to services in the community and set goals to reach a place of stability. 

Offers financial literacy counseling to help youth gain tools to help them become economically self-sufficient. Children learn how to reduce debt, build savings, improve credit, create a budget, and develop a plan to accomplish financial stability.

Provides employment services for those who are dislocated workers. Individuals may be considered dislocated workers if they match one of the following situations:

- Lost employment due to a permanent plant closure or mass layoff within the company.

- Position has been eliminated and unable to return to work in that industry or occupation.

- Self-employed, but the economy or a natural disaster is causing the business to fail.

- Displaced homemaker or caregiver who has lost their primary means of support.

- A military spouse, who left a job to follow the military service member, or unable to continue employment because a spouse's change of military station, or lost employment as a result of the military member's discharge from the military.

Offers to empower male youth to make positive life choices. Program provides participants with mentor/mentee matches, weekly group mentoring sessions, graduation ceremonies and special events. Locations vary within the Polk County area, including community and school-based sites.

Provides day habilitation and community integration services to build and improve skills for daily living, self-care, social skills, and community living.

Summer 2026:

Starting June 3rd through Aug 5th, children and teens can receive free meals throughout the summer by visiting one of the sites throughout Lee County or Ogle County. No registration or ID is required.

Lunch-serving sites are open on Wednesdays, providing three grab-and-go meals for each child.

- Old Lincoln School: 12 pm-12:30 pm.

- St Paul Lutheran Church: 12 pm-12:30 pm.

- Wooden Wonderland: 12 pm-12:30 pm.

- Splash Pad at Vaile Park: 12 pm-12:30 pm.

- Dixon High School: 12 pm-12:30 pm.

- Jefferson School: 12 pm-12:30 pm.

- Washington School: 12 pm-12:30 pm.

Nutrition education to adults and youth of limited resources about healthy eating and food resources to improve food resource management skills. The program offers lessons in the areas of diet quality, food safety, and food resource management individually or in small groups.

Non Emergency Medical Transportation Provider for Medicaid members and self-pay clients. Taxi Service is available by appointment for rides locally or further.

Provides information and support for parents. Home visits are available by trained parent educators. Group meetings with other parents are also available.
