Provides a food pantry. 

Provides rent, utility, or medication assistance. Does not assist with past due rent/utilities or rent/utility deposits. Does not assist anyone eligible or receiving FIP. Repayment agreement is required.

Interim Disability Program assists adults who are 100% disabled as deemed by their doctor and have zero income and resources in the household. All members of the household must meet requirements to qualify for this program.

Back to Work program provides assistance for up to 3 consecutive months not to exceed a fixed limit for a 1 month period. Clients must complete 40 volunteer hours and submit 8 job applications per month.

One-Time Needy Assistance Program provides assistance once within a Fiscal Year with the amount of assistance not to exceed a fixed limit; clients must have a plan to pay future bills.

Provides a Burial Assistance Program; approved person must have been previously residing in Pottawattamie County but considered homeless, i.e. indigent, transient.

The General Assistance Program is to be considered the agency of last resort; all callers will be directed to contact other assistance agencies prior to funding consideration.

Wayne County Fire Response can provide contact information for all fire and emergency response units in Wayne County dispatched through the Wayne County Sheriff's Office. In case of emergency call 911.

Subsidized housing for persons ages 62 and older or mobility-impaired. Meal site and laundry facilities on-site. Utilities are included in rent. Offers social programs, professional management, maintenance, and service coordinator.

This is a four-story building that provides hot water baseboard heat. Each apartment has a small deck.

Provides cancer care treatment to those in need. This is one of four cancer centers that now offers the Brachytherapy treatment option, which is targeted cancer treatment that focuses on sending a beam of radiation directly to the cancer cells. 

Helpline offers a gambling assessment.  Provides individualized treatment plans for those struggling with a gambling problem. The intensity of treatment, the length of treatment, and the goals to be achieved are based on individual client need and agreement.  

Provides a food bank that offers food and other material goods to non-profit organizations, who then distributes these goods to individuals in need.

Provides health care services to those in need. Services include sick care, chronic disease management, preventive care services, cervical cancer screenings (Pap smears), physical  exams, vaccinations, sexual health services, STI testing and treatment, wellness checkups, work physicals, laboratory and diagnostic testing, referrals for mammograms and other specialty care.

Strives to enhance the mind, body, and spirits of individuals with disabilities served through the following programs: - Developmental skills.

- Exercise and nutrition.

- Classes of varying topics.

- Community outings.

- Adventure For All.

Provides assistance for youth, ages 17 up to age 23, who are preparing to exit or have 'aged out' of foster care or other court ordered placement, meet the challenges of living independently and transition successfully to adulthood.Case Management provides coordination and linkage to available community resources.

Services include life skills training and experimental learning opportunities, assistance in obtaining and maintaining a safe and stable place to live, assistance in identifying and achieving personal education and employment goals, access to financial assistance to meet emergency or short-term needs, and assistance in obtaining medical care and mental health services.

Provides child vaccines. All scheduled childhood vaccines are available. Immunization for infants and children (6 weeks to 18 years of age) for uninsured, underinsured, and Medicaid/All Kid's recipients. 8 clinics per month, 6 total sites located throughout McHenry County

Adult Vaccines, all recommended vaccines for adults (19 years of age and older) for both uninsured and underinsured. 2 walk-in clinics per month. Travel vaccines are not offered.

Provides supervision for delinquent youth under 18 who are involved with the court and or probation systems.

Offers people experiencing a mental health episode an alternative to hospitalization. Services include crisis support, stress relief, coping skills training,  and community resource referrals.

Provides a place for those with hearing loss, or for those with difficulty hearing, to socialize with others like them. 

Provides eye exams, hearing screenings, and/or new eyeglasses. Also accepts donations of old glasses and hearing aids.

Provides mentoring services to youth. Services include weekly group mentorship sessions, guest speakers, leadership activities, and community engagement opportunities. The mentorship initiative focuses on helping youth navigate challenges, set goals, strengthen self-esteem, and develop leadership qualities.

Provides in office or telehealth therapy at all locations. Can also provide in-home or in-office behavioral health services for children, ages 4-18 and their families. BHIS services can also be done via telehealth as well.

Offers home health care and hospice services. Care may include:

• Regular visits to patient's home to provide care.

  - Coordinate services needed from other healthcare providers, including doctors, therapists, social workers and nurse aides.

• Keeps family and primary care physician informed about progress and care plan.

  VNA will also answer questions about hospice care or end-of-life questions, even if a patient is not ready for hospice care at this time. Topics include:

• Advance directives.

• End-of-life issues.

• Making plans to provide care for a loved one with a terminal illness.

• Grief and loss issues.

• Understanding palliative care vs. curative treatment.

Counseling can help strengthen your family, develop your relationships and prevent small problems from getting out of control. Individual, family and group therapists assess and treat everything from everyday stress to chronic mental health disorders.

Outpatient substance use disorder services are available for persons who have used drugs or alcohol or have been diagnosed as chemically dependent, if they can be treated in the community.
