Renewal forms available for clients to fill out and send in themselves. PLEASE NOTE: All minor children (regardless of age) must appear in person when applying for a passport. Children under the age of 16 years must be accompanied by both parents or the child's legal guardian. Notarized consent is required from the non-applying parent if only one appears with child under the age of 16.
