Social Assistance Workers are available to help patients access resources, address urgent financial issues and work through housing problems, violence, sexual assault and other issues with the goal of promoting self-efficacy in all areas of life that impact health and well-being.

Guides families through their child’s journey with a medical condition. Works with doctors, schools and community groups to create a seamless support system.

Care coordination team works together with families to develop a plan of care that addresses a child’s medical, social, behavioral, educational and financial needs. Care coordination is free for all children who have eligible medical conditions, regardless of their family’s income level.

Care coordination can include finding specialized medical care, helping families understand their child’s diagnosis and medical treatment plan, explaining insurance benefits and attending school meetings.

Helps children who are medically fragile remain in their family home rather than a hospital or skilled nursing facility.

Assists with:

- Finding specialized medical care

- Making sure families understand their child’s diagnosis and medical treatment plan

- Helping families understand their insurance benefits and maximize their coverage

- Attending IEP /504 Plan school meeting

- Connecting families with financial support, grants and other community resources

- Preparing for the transition to adulthood

Provides direct services to victims/survivors of domestic violence, sexual violence, dating violence, sexual harassment, and human trafficking in the Asian Pacific Islander communities in Iowa. Services include domestic violence and sexual assault hotlines, adult and youth domestic violence and sexual assault prevention and intervention programs, spouse and intimate partner abuse prevention, and child sexual assault prevention programs that educate children about personal safety, recognizing inappropriate behavior, and reporting abuse.

Offers Beggar's Night for the City of Grimes.

Provides financial help to individuals who do not live in Bloomington who are passing through the city of Bloomington to get to the next largest town or city. The most inexpensive form of transportation will be used. If traveling by car, assistance may be a voucher for gasoline.

Offers a food pantry that provides a box of food for families and individuals in need.

Provides temporary financial assistance for pregnant women and families with one or more dependent children.

TANF provides financial assistance to help pay for food, shelter, utilities, and expenses other than medical.

Transitional services are also available to help families become independent, such as GED preparation, vocational training, job retention programs, etc.

Offers a range of services to address academic, social, emotional, and behavioral concerns of children and adolescents. Services include comprehensive evaluations, counseling/therapy, parent and school consultation, academic intervention with children and youth. The site also completes autism diagnostic assessments.
