Provides outpatient mental health therapy, medication management, and psychiatric assessments. Services are offered to adults with disabilities, including individuals with Mental Illness, Intellectual Disability, or other Developmental Disabilities, who require support in multiple areas. 

The program integrates Medicaid funding and county regional funding to assist individuals in transitioning back into the community after hospitalization for acute or chronic mental or emotional disturbances. 

Eligibility requires qualification for Polk County Regional funding; individuals may or may not be eligible for Medicaid.

Offers to help children in grades 4-12 develop knowledge and skills. Services include learning about citizenship, leadership, cooking, arts and crafts, mechanics and technology, horticulture, agriculture, and other subjects. To teach young people about these subjects, the 4-H program uses the learn-by-doing method of instruction.

Offered to any adult with mental illness living in Iowa. Peer support specialists offer emotional support, help with resources and referrals, and goal-setting over the phone, text or Zoom. Sign up for a weekly 30 minute slot and receive peer support from home.

Provides help to children who are likely to experience multiple placements or being in an institution, then places them with specially trained foster parents to receive these supports in a home setting. Offers daily care and guidance from trained foster parents to help prepare the child for reunification, a traditional foster home, or adoption.

Offers yoga classes for free to individuals currently undergoing cancer treatment as well as cancer survivors and YMCA members. Services include a range of class styles for all experience levels, with options for low, medium, and high intensity. 

Offers to assist in helping eligible Native Americans pay for childcare.

Inspections of all restaurants, food stores, processors and warehouses regarding sanitation, hygiene and food protection. Technical assistance is provided, including the review of plans for new establishments to insure proper equipment and construction, and the Hazard Analysis Critical Control Point (HACCP) review of food preparation. Complaints are investigated, including those involving potential foodborne illnesses. The Program is advised by the Food Advisory Committee, which is composed of industry representatives and other citizens.

Provides food handler/manager education and permitting.

Offers a senior center. Services are open five days a week for recreational opportunities, including games, newspapers, socialization, and speakers.

Community Christmas Family Sign-Up is open as of Oct 15, 2025. Families can receive Christmas gifts, books, games, clothing, and food for the holidays.

Prenatal care, preparing for birth and questions and answers about breast feeding. Expectant moms are encouraged to bring support person.

Provides a rent subsidy program. This program is similar to the HUD voucher program, that targets extremely low-income renters and helps them pay their monthly rent.
