Community Support Advocates

The program integrates Medicaid funding and county regional funding to assist individuals in transitioning back into the community after hospitalization for acute or chronic mental or emotional disturbances.
Eligibility requires qualification for Polk County Regional funding; individuals may or may not be eligible for Medicaid.

Provides help to children who are likely to experience multiple placements or being in an institution, then places them with specially trained foster parents to receive these supports in a home setting. Offers daily care and guidance from trained foster parents to help prepare the child for reunification, a traditional foster home, or adoption.

Inspections of all restaurants, food stores, processors and warehouses regarding sanitation, hygiene and food protection. Technical assistance is provided, including the review of plans for new establishments to insure proper equipment and construction, and the Hazard Analysis Critical Control Point (HACCP) review of food preparation. Complaints are investigated, including those involving potential foodborne illnesses. The Program is advised by the Food Advisory Committee, which is composed of industry representatives and other citizens.
