Provides services to people who have advanced, chronic or life-limiting illnesses. Services include:

• In-home pain and symptom management.
• Care coordination with current healthcare team.
• Guidance with the development of the patient's individual plan of care.
• Advocacy for the patient.

Administers probation supervision, pre-trial release, pre-sentence investigation, and parole services, including programs that oversee individuals under legal custody and facilitate movement between jurisdictions or facilities. 

Conducts bail hearings to determine release conditions, bail amounts, or detention status for individuals awaiting trial. Utilizes electronic monitoring technology, such as transmitters and location tracking equipment, to monitor offenders confined to their homes or awaiting judicial procedures. 

Offers low-risk probation for minimum supervision offenders with individualized court-ordered conditions to be completed within a designated timeframe.

Helps people with little income buy food for a healthy diet. An application for Food Assistance must be completed to receive this assistance. Call or visit website for more detailed information.

Offers a multispecialty practice providing diagnosis, treatment, and ongoing care for a wide range of acute and chronic illnesses and injuries.

Offers access to informational, educational, and recreational resources to the community, including online resources, genealogy, and language learning. Services also include free access to computers, Wi-Fi, meeting spaces, and events for all ages. 

Provides a food pantry for those in need. USDA government commodities are distributed along with purchased and donated food, including weekly donations of perishable items from Trader Joes.

Provides a weekly warehouse grocery store pantry. Available pantry items may include, perishable food products, non-perishable food products, fruits, vegetables, breads, toiletries, cleaning products.

Provides support to cancer patients and their caregivers through programs and services. Services are in support of the survivor's quality of life and include physical, psycho-social and nutritional services.

Provides medical appointment transportation for seniors. Other transportation needs are not covered at this time. Transportation is available for trips both within the county and also into surrounding areas outside of the county.

Offers business and Industry Loan Guarantees that are available for business and industrial development in rural areas to create and maintain employment, expand and to improve economic climate. Eligible borrowers include Public bodies, Community-based non-profit corporations, Federally-recognized Tribes Funds can be used to purchase, construct, and/or improve essential community facilities, purchase equipment and pay related project expenses.

Provides clothing vouchers to those in need, to be used at the Salvation Army Thrift Store. 

Offers a program that helps you to save time and money through planning your meals and shopping strategies. Programming and materials may also be available online. 

Offers both short and long-term experiences for youth. Short-term experiences include afterschool programs, camps, school-sponsored activities, conferences, and other events. Long-term experiences include learning communities in agriculture such as county clubs.

Provides guidance and support to families of individuals with Down Syndrome, through speaker events, newsletters, social events, community awareness activities, and a resource library. There is no permanent location; meetings are primarily held in the Crystal Lake area.

Providers certified orientation and mobility specialists who teach individuals to use a white cane and navigate their surroundings.

Offers a Fresh Food Counter, open to anyone during business hours.

- Fresh produce, dairy products, rice, Meals from the Heartland, and beans are available while supplies last.

- Bread is available every day for families in need, regardless of their address. Families may take up to two loaves per day per household.

Nutritious meals and socializing for older adults ages 60 and older.
