Focus program partnership between Lincoln Public Schools and The Bay, providing high school juniors and seniors with the tools, access, and resources to be successful creators (music, video content, art, and more).

Provides health care services in the home by registered nurses. Services include infusion therapy, central venous catheter care and injections, wound and catheter care, gait training, and health education. Also offers physical, occupational, and speech therapists, medical social workers, certified nursing assistants, and telehealth nursing.

Pediatric Home Nursing: Provides care according to the patients’ needs and the doctor’s orders, including both extended stay and skilled visits. Provides in-home care for complex pediatric clients such as those using ventilators, IV and TPN needs, trachs, g-tubes, central lines, respiratory medication, and monitoring equipment.

Health-Related Clinics: Offered in various locations, including seasonal influenza and pneumonia vaccinations, foot-care clinics, and community wellness screenings.

Hospice Care: Provides medical care, symptom and pain management, and support to persons with a limited life expectancy and their families. Registered nurses, chaplains, home health aides, volunteers, social workers, and homemakers provide service in the home or in a residential hospice facility setting.

Palliative Care: Provides pain and non-pain symptom management for patients with serious and life-threatening illness. Consultative services are provided with care centered on the needs of the patient and family, providing counseling about the disease and assistance in identifying the patient's personal goals for care.

Provides rental assistance, available once every 2 years.

Provides transportation services to multiple counties.

Riders are encouraged to schedule trips at least one working day in advance (same-day service is available if space permits), and riders must be ready for pick-up 15 minutes prior to the scheduled pick-up time. Cancellations must be made at least two hours prior to pick-up time.

All vehicles are wheelchair accessible and ADA compliant.

Offers to help patients prevent illness, manage ongoing health conditions, and get care when they are sick. Services may include exams, screenings, immunizations, care for acute illness, chronic disease care, referrals, and care coordination.

Offers a home hospice program for patients and families throughout central Illinois. A coordinated care team visits individuals and their families in their own home. "Home" can be anywhere an individual calls home - house, apartment, nursing or retirement facility. The goal is to keep patients in familiar surroundings and close to loved ones for as long as possible.

Offers an exclusive online job, apprenticeship, internship, and career portal. Post resumes and portfolios so employers can view. Search and apply for current job openings. Learn about on-campus recruiting visits and job fairs. Employers post their job openings and can search for candidates.

Offers support groups for seniors and their caregivers. Support groups being offered are Alzheimer's Caregiver Support Group (meetings held in Normal and Clinton), Grandparents Raising Grandchildren Support Group (Zoom and in-person meetings held in Bloomington and Pontiac), and Caretakers Path Caregiver Support Group (Zoom and in-person meetings held in Bloomington).

Provides a community and home-based therapeutic mentoring program to youth who are wards of the state and are identified by their DCFS/POS caseworker as needing more intensive services to stabilize their current placement in foster care.

Congregate and home delivered meals are served in Polk and Shelby on weekdays. Home delivered meals are distribed in Osceola on weekdays and Stromsburg every day of the week.

Centers have holiday and special meals as well as potluck meals once per month. Exercise and nutrition programs are available as well as free blood pressure checks by Polk County Health Department. Pool and pitch tournaments take place monthly.

Polk County and Aging Partners in Lincoln help to provide Information and Referral services.

Provides case management for budgeting, money management/payee services,  and nursing home placement for the mentally ill. Also helps to provide linkage following discharge from both state operated facilities and hospitals.

Offers Narcan training at various sites around the county or at the individuals place of business. The class teaches individuals how to handle and administer Narcan and after receiving the 30-minute training individuals will be given a box of Narcan. 

Provides community-based residential services, including housing, supervision, and programming, for adults ordered to reside at the facility as a condition of probation, parole, early release from prison, or a prison sentence for a drunk driving offense. The facility is operated by the Sixth Judicial District Department of Correctional Services.

Academic assistance program funded through TRIO Grants by the U.S. Department of Education. The program is designed for low-income, first-generation college students or students with a disability. Participants will have the opportunity to meet with their TRIO/SSS advisor once a month, and are encouraged to participate in one of the various program activities each month.

Individual, family, and group therapists assess and treat everything from everyday stress to chronic mental health disorders.

Outpatient substance use disorder services are available for persons who have used drugs or alcohol or have been diagnosed as chemically dependent, if they can be treated in the community.

Specialized treatment services and groups include:

- Severe and persistent mental illness

- Depression and mood disorder

- Urgent outpatient mental health care

- Eye Movement Desensitization and Reprocessing (EMDR)

- Counseling for sex offenders

- Treatment for children with sexual behavior problems

- Sexual abuse support groups

- Substance use disorder

- Adolescent decision-making groups

- Post-adoption issues

- Pregnancy- and parenting-related issues

- Coping Skills Groups

Provides non-medical assistance that helps older adults maintain their independence, comfort and quality of life in their own home. Services include light housekeeping, laundry, meal preparation, shopping and errand running, personal grooming and bathing, and escorting clients to doctor appointments through the transportation services provided.

Provides support, mentoring, community services, parent workshops for LGBTQ+ youth, families, and allies.

Provides comprehensive dental care, including Oral exams, Cleanings, X-rays, Sealants, Fillings, Extractions, Crowns, Dentures and partials, Referrals.

Provides DUI Evaluation, DUI Risk Education, Intervention Classes, and Basic and Intensive Outpatient treatment services for substance use disorders.
