Helps consumers prepare electronic and paper applications to establish eligibility to enroll in coverage through the Health Insurance Marketplaces and potentially qualify for an insurance affordability program. Counselors are trained and certified to help individuals understand health coverage options and enroll in a plan.

Provides home health care services that allow individuals to continue living in the home while receiving medical care. Offers skilled nursing and home health aides, as well as physical, occupational and speech therapy. A medical social worker can also connect individuals to the resources they need.

Provides food, clothing, personal hygiene products, and household goods once a month per certified client/family. Online ordering is available.

Provides information and assistance regarding programs and services available for older adults, individuals with disabilities and caregivers. 

Responsibilities:

- Prosecutes all state criminal cases (felony and misdemeanor) and traffic cases for Moultrie County.

- Provides the legal counsel for county business.

- Assists those petitioning for Orders of Protection.

- Goes to court to collect unpaid child support.

- May assist with victim restitution and plea offer requests.

This office does not represent individual citizens.

Provides self-care skills, fine and visual motor skills, behavior, accessibility, positioning, play and leisure skills, work and vocational skills, and social skills.

Provides weatherization services to make homes more energy efficient through health and safety checks including furnaces, insulation, water heaters and sealing doorways and windows. Inefficient heating components may be repaired or replaced.

Provides confidential outpatient counseling services. Counselors take a holistic approach to treatment, addressing mental, physical, and spiritual conditions.

- Assists individuals, businesses, and other non-profit organizations to establish permanent endowment and temporary funds, through gifts and bequests, within the confines of one large foundation.

- Foundation funds are used to provide grants to non-profit organizations and scholarships to students within the Fox River Valley, including the Greater Aurora Area and the Tri-Cities.

Emergency housing offers a safe haven for persons in need. The shelter provides a warm bed, three meals per day, shower and laundry facilities, personal hygiene items, clothing, and supportive services to link homeless individuals with case management. Guests may also ask for help with GED tutoring and employment services.

Offers heart health care to those in need. Services can include cardiovascular surgery, clinics, rehabilitation, therapy, diagnostic imaging and more.

Offers sleep disorder services and treatments for anyone having issues with sleep. Sleep studies are conducted to gather information for diagnosis and to create a treatment plan that will help with the sleeping disorder.

Aftercare/Continuing Care for people who have completed primary treatment and need some additional support.

Law enforcement services.

Offers a program for parents and youth individuals ages 10-14 to work together and build relationships. Participants will practice skills, play learning games, and do family projects in highly interactive sessions. Sessions are designed to increase family bonding, build positive communication skills, and solve problems together. 

Offers Basic Skills/GED® classes, English as a Second Language instruction, and programs that prepare individuals for U.S. citizenship by fostering an understanding of American culture, history, government, and civic responsibilities. Volunteer opportunities are also available.

Offers to help individuals learn and keep important life skills so they can be part of the community. Day habilitation takes place in small groups. Each individual must arrange their own ride to and from the meeting place each day.

Provides dresses to girls/women in need of a dress during prom season only.

Offers a one-day event open to all high school girls. They are able to get the dress of their dreams, accessories, and be pampered by a personal shopper.

- Independent living skills training that will enable an individual with disabilities to remain in their home, or when transitioning into their first independent living.

- Advocacy for individuals and systems that serve them.

- Driver's Education School is DMV certified; provides evaluation of reaction time, visual perception, and behind the wheel skills. Education and training includes classroom and on-the-road instruction. Offers adaptive equipment evaluation and training.

- Durable medical equipment rent/loan at Grand Island and Scottsbluff locations. Donations of equipment accepted.

- Peer mentoring, where people with disabilities support each other in their daily lives.

- Housing counseling assists people with disabilities in locating, acquiring, and maintaining safe, affordable, accessible housing.
