Provides assistance for Nebraskans in accessing services and support to help meet their long-term care needs. Services include living arrangements related to long-term care, in-home services such as housekeeping, personal care, respite, equipment, care coordination, paying for long-term care services, health promotion and prevention, mental health services, financial guidance and resources, as well as other basic needs including food, shelter, Medicaid and Medicare.

- Ray and Kathy LaHood Center for Cerebral Palsy: Variety of diagnoses that impact movement and motor development, including but not limited to cerebral palsy, hypotonia, developmental delays, and genetic disorders. May assess the child's range of motion, gait pattern, orthotics, and other assistive technology equipment, review the child and family's needs, and work with the family and physician to establish a plan of care.

- Orthotics Clinic: Prescription for custom-fitted orthotics/braces to provide stability and support for proper positioning of the feet and ankles during standing, walking, and other functional activities.

Provides help to residents locating resources and services, explore options and navigate systems. Can assists in health-related areas: access to and payment for medical care including medications, mental health/substance abuse, parenting/childcare, aging issues, housing or food.

Provides autism screenings and help find diagnostic services. Teams help Iowa families navigate local supports and services for children and youth with autism spectrum disorder (ASD) and provide family-to-family support. 

Offers project-based housing.

Provides a Community Christmas Store - Children ages birth to fourteen are eligible. Parents pay $5.00 per child to shop. The store will feature NEW clothing, winter apparel, blankets, books and toys for the parents to select from.

Christmas Baskets - Families receive all the ingredients for a traditional holiday meal.

All children, ages birth to twelve, from qualifying families for both programs, will have their names placed on the Angel Tree at Midwest Bank.

Applications are accepted between October 1, 2026, and the day before Thanksgiving 2026.

Provides a licensed substance abuse assessment and evaluation program. Offer affordable, confidential substance abuse (drug or alcohol) evaluation services for court ordered and voluntary clients. Also offers OWI weekend classes for 1st and 2nd OWI offenses.

Offers an assisted living memory care facility with 16 private apartments with showers. The assisted living memory care facility offers memory-enhancing treatments and activities facilitated by dementia care professionals.

Activity and sensory stations allow for personalized activities for older adults, to help provide an environment where older adults are engaged and participating in tasks that are meaningful to them and contribute to their neighborhood.

Provides financial assistance and education to eligible households with a grocery voucher to meet their meal gaps.

Senior services include setting up and administering medications, monitoring blood pressure and blood sugar, nursing care, personal care, and companionship.

Provides free counseling services for state of Illinois residents either in-person or via tele-counseling (using video conferencing).

Provides remedial services that offer in-home skill development services to children who are eligible for Title 19 and have a mental health diagnosis to enhance functioning. Services provided to children and families include; conflict resolution, problem solving, anger management, coping, social interaction, self control, crisis intervention, and communication skills.

Assists low-income homeowners and renters pay for a portion of their primary heating costs for the winter heating season. Assistance is based on household income, size, type of fuel and type of housing. The application period is from October 1-April 30 for elderly and disabled households and November 1-April 30th for the rest of the households.

LIHEAP also offers a crisis program that may be able to assist homeowners who are with no heat or limited heat. The furnace component for crisis runs October 1-April 30th.

Offers free or low cost mammograms available for eligible women. Walk-in appointments are available. Obstetrical services/OB  Services. Midwifery services are also offered. Family planning offers examinations of women's reproductive systems, pregnancy testing, information and testing for sexually transmitted diseases are available.

Provides information on Parkinson's disease, including its causes, symptoms, treatment options, and therapeutic activities to do at home. Also offers resources and insights to foster understanding and awareness about living with Parkinson's disease, promoting positive communication and interaction with individuals affected by the condition.
