Family Support Workers partner with parents to enhance protective factors within their families through the use of evidence based practices to achieve family permanency, safety, and well-being.

Provides Parenting Time/Supervised Visitation, Family Support Nebraska, and drug testing.

Intensive residential treatment with clinically managed high-intensity residential services including 24-hour monitoring and intensive treatment. Offers group treatment sessions, peer support groups, individual sessions, case management and other recovery support activities throughout treatment. Provides extended residential care with separate residential units for men and women in recovery.

Provides transportation to participants, ages 60 and over, as well as individuals ages 18 and over with a disability, living in Sangamon County. Vehicles are wheelchair accessible. Reservations are required.

Strategies to Embrace Living with Lupus Fearlessly (SELF) is a free online self-management program designed to help people with lupus live their best life. SELF is tailored to support people with lupus in managing symptoms, stress, medications, and working with their healthcare team.

National Resource Center on Lupus offers a collection of up-to-date resources and information on lupus. The Resource Center educates, empowers, and connects people to lupus resources, programs, and support services.

Health Education Specialists are available to answer questions and provide caring support to people with lupus. Health education specialists are specially trained to provide people affected by lupus with non-medical counseling, disease education, information, and helpful resources. Bilingual (Spanish) health educators are also available.

The Expert Series is an educational podcast that features empowering conversations with leading lupus experts that focus on helping people live well with lupus.

LupusConnect is an online community where people affected by lupus can engage with others to share experiences, find emotional support and discuss practical ways to live and cope with the disease.

Take Charge is a weekly education email series designed to give people with lupus, including those recently diagnosed, tips and resources that can help them better manage their lupus and its everyday challenges.

Lupus and You: Answers. Advocacy. Action. is a free educational series where people with lupus, their family, and friends have the opportunity to learn about the latest in lupus research and helpful resources for managing the disease and hear about ways to engage in advocacy and awareness efforts.

Local Support and Programs – There are opportunities to engage locally and participate in education events and support groups dedicated to improving the lives of people affected by lupus. On-the-ground experts can help find resources, and support groups, and offer ways to get involved in the fight to end lupus.

Provides case/care management assessments for seniors who may need homemaker services.

Helpline staffed by nurses, social workers and therapists. Services include:

- Current disease information.

- Medical issues including symptoms and treatments.

- Health and social care.

- Emotional support.

- Referrals to health professionals and community resources for local support.

Free and confidential treatment referral and information about mental and/or substance use disorders, prevention, and recovery.

Public health works with older adults and Elderbridge Area Agency on Aging. 

Connects trained community health workers (CHWs) to community members to help coordinate medical and social resources needed to be healthy and thrive. With a current focus on pregnancy and birth outcomes, CHWs coordinate needed medical care (ex: prenatal care, healthcare coverage, dental care) and a large range of social needs (housing, employment, transportation, and more) and also deliver evidence-based health education to increase knowledge and behaviors that promote a healthy pregnancy and postpartum period.

Direct services are provided through Charles Drew Health Center, CHI Health, and A Mother's Love. Services are not offered directly through the Omaha Community Foundation.

Provides support through Thrive Iowa, a community-led initiative that uses a collective impact model to help families move from crisis to stability. Uses resource navigation, community partners, and a collaborative case management system to connect families with needed support.

Case Management services to caregivers age 18 and older, living with and providing daily care to a care receiver age 60 or older or with a dementia-related diagnosis.

LIHEAP (low income home energy assistance program) provides assistance with a utility bill. Applications are accepted November 1 – April 30; for elderly or disabled individuals applications are accepted starting October 1. Income must fall within 200% of poverty guideline. Call for an appointment. A letter has been sent to those who may be eligible for early application in August or September.

**This program is not designed to pay a household's total energy costs. The program will provide supplemental assistance based on several factors. This is a one-time payment to your energy provider.

Offers programs to assist families and individuals in rural America with financing, mortgage payment assistance, and opportunities to buy, build, repair, or own safe and affordable homes. 

Additionally, the service provides access to information about privately owned rental housing options available at below-market rates, including government-subsidized housing and low-income/subsidized private rental housing. These housing options may include units developed or improved with public funds, income-based rental housing, and fixed below-market rate rental housing, benefiting low-income households, older adults, people with disabilities, and other special populations.

Works with disabled individuals to find employment by assisting them with job readiness skills, benefits counseling, job development, and case management. The program works with local business, as well as state and federal agencies, to ensure that the services are all encompassing.

LIHEAP (Low Income Home Energy Assistance Program) provides supplemental help for the winter heating bills November 1st through April 30th: LIHEAP Crisis Assistance when funding available provides in a life-threatening situation fuel, heaters, furnace repair; furnace replacement for homeowners when funds available. Applications are accepted October 1-April 30 for elderly or disabled households and November 1-April 30 for all other households. May have some crisis funding available outside the normal LIHEAP months, call local County Outreach Office for availability of funding.
