PACE (Program of All-inclusive Care for the Elderly) organization offering a complete personal health plan for eligible individuals. Supports living in the client's own home and community for as long as possible.

Participants receive all the care and services covered by Medicare and Medicaid, as authorized by the Interdisciplinary Team, as well as additional medically necessary care and services not covered by Medicare and Medicaid.

-- Home safety products (grab bars, bath/shower benches, elevated toilet seats)

Provides a food basket to those in need. Can be accessed every 30 days as needed.

Provides free mammograms for women ages 40 to 64 and free clinical breast/pelvic exams, Pap tests, and HPV tests for women ages 35 to 64 who are uninsured or underinsured. This program includes case management, education, and outreach efforts.

Offers books for children and adults, a wide range of magazines, audio books, videos, 4 computers; 2 of which are Internet accessible, Microfilm files of Old Brooklyn newspapers, Children's room, and Conference room.

Donations of new and gently used coats and other winter wear items are being accepted. All sizes from infant to adult are needed.
