Homeless Veteran Supportive Services | Primary Health Care

SSVF is a Veterans Affairs funded program which is delivered through Primary Health Care. This program is focused on helping homeless and unstably-housed Veterans and their families living in Warren, Polk, Dallas, Jasper, Story, and Marshall Counties. Services provided include:

CASE MANAGEMENT services that will assist participants in obtaining VA and other public benefits, including: health care referrals, daily living referrals, personal financial planning, fiduciary and payee referrals, legal services, housing counseling services, employment and training referrals.

TEMPORARY FINANCIAL ASSISTANCE (TFA) which may include rent assistance, moving expenses, security and utility deposits, transportation, and child care. All financial services based on eligibility and available funding.

Provides moving and storage services to Defense Department and Coast Guard personnel and their families. Individuals and families can browse resources, fact sheets, and customer service pages to prepare and implement upcoming moves.

Supportive Services for Veteran Families | Hawkeye Area Community Action Program (HACAP)

Funding provided by the VA to improve the housing stability of low income Veterans. May be able to assist in moving expense assistance for homeless Veterans in need.

Financial Services | Cross Outreach

Cross Outreach is a homelessness prevention program offering limited assistance with basic needs for households in transition, subject to available funding. Services include help with rent, security deposits, electric or water bills, laundry vouchers, storage costs, and support for moving-related needs. While assistance with car gas is not provided, the program also offers resources to help individuals maintain clean and sanitary living environments.

***Rent, security deposits, electric, water and laundry voucher assistance requires clients to email a help request on the 3rd Monday of every month between 8 am-8 pm.

2026 Intake Dates are always on the 3rd Monday of each month, 8 am-8 pm.
