Provides temporary homes for children of families in crisis. Services allow parents an opportunity to resolve a temporary crisis while reducing risk of abuse, neglect, and isolation for children. Connects families with a family friend, who offers a healthy relational support as the family navigates the situation. Also connects the family with a family coach, who can provide linkage to services in the community and set goals to reach a place of stability. 

Holiday baskets are offered seasonally, provided through local community support. Programs vary by location; call for information.

Provides anger management classes, including domestic violence classes. Certificate provided upon completion.

- Minimum class hours are 4. Does not include intake. Anger Management can be completed as individual or group sessions.

Family resource program to support parents in educating and nurturing their children. Health and mental health services are also provided. Helps parents better understand their child’s stages of development with individualized visits in the home.

- Weekly visits are 90 minutes and are designed to meet the needs of each family.

- Staff will assist families in planning activities that encourage the child’s growth.

- Social activities will be held twice monthly, to interact with the child and other families.

Provides primary care across the patient’s life span, including family medicine, internal medicine, infectious disease management, pediatric care, OB/GYN services, prenatal and postpartum coordinated care with labor and delivery, immunizations and preventative screenings, and chronic care.

Offers clothing vouchers as available, food pantry as available, and computer access.

Provides a meal, social activities and educational programs for older adults ages 60 and older. Services also include a food pantry for residents in the English Valley School District. The center may also be available to rent Mon-Sun for parties, social events, reunions, fundraisers and meetings.

Focuses on improving the nutrition, food safety, and physical activity of Nebraska's youth and adults. Includes diabetes education.

County level services provide direct emergency economic aid for vital necessities to veterans, their widow, and dependents, according to their needs. Personal visits for assistance and claims to housebound veterans or dependents. Must have an honorable or under honorable conditions discharge from active duty or wartime service. Must be a resident in Nebraska for the past one year and 6 months of that year in Polk County.

State level services do not require wartime service. Assists in completing applications for the Nebraska Veterans Aid Fund, fee exempt fishing and hunting permits, applications to the Nebraska Veterans Homes, and waivers of tuition.

Federal level services provide and/or complete Veteran Administration forms for claims relating but not limited to service connected compensation, disability pension, widow's pension, burial allowance, grave markers/headstones, educational benefits, home loans, insurance, indebtedness, and character of service upgrades.

Provides law enforcement services.

Provides practical information for anyone to access in the areas served. Education services include agriculture, horticulture, family and consumer economics, nutrition and wellness, and youth development. The website has information about different programming options available.

Non Emergency Medical Transportation Provider for Medicaid members and self-pay clients. Taxi Service is available by appointment for rides locally or further.

Senior centers provide a place for congregate meals, as well as social activities, games, educational programs, etc.

Provides a food pantry to those in need every 2nd and 4th Wednesday of the month from 4:30 pm-5:30 pm. 

Vital Records (birth, death, marriage, civil union, etc.), Property Taxes (maintains property tax information), Election authority, and County Board.

Offers an 8-session course for adults experiencing mental illness(es), including substance use disorder. P2P is taught by "Peers" (adults with mental illness). Two or three trained Peer Instructors volunteer to teach the course. Peers learn about common mental illness diagnoses, effective ways to deal with them, and how to self-advocate to get the best care.
